Statistical Study on the Impact of Computer-use on Child-health in the Arab-community

United States of America (USA), Australia, and Canada recommending children computer-use should be imperfect. It obligated a strong influence on children’s lives. The problem was for stuck in the computer-use for a long time per day. The goal was to follow child-health in the Arab-community to notice its influence on the child-health. The method was through a arithmetical study by sending Internet questionnaire and receiving the parents' advice. The results were in the 500 parent answers and 14 comments. The first was "You have a child up to 13 years old?"; 83.6%. The second was "Your child used a computer?"; 80.8%. The third was "Your child used the computer for a long time?"; 67.9%. The fourth was "The computer caused a health problem for your child?"; 70.3%. The fifth was "The computer caused your child mental disorder?"; 68.5%. The sixth was "The computer caused your child trouble seeing?"; 77.2%. The seventh was "The computer caused your child trouble sleeping?"; 72.0%. The eighth 8 was "The computer caused your child feeding problem?"; 69.5%. The ninth was "The computer caused your child family relationship problem?", 72.0%. The tenth was "Prevent your child from using the computer to protect his health?"; 79.0%. It was concluded that the extent of the harms of child computer-use from a health and psychological point. They have young and sensitive tissues. The computer-use affected them, child-health. It was recommended the parents instructions will protect children from the computer-use for long periods. That will reduce child-health and psychological problems.

Design of Stopwatch based on STC89C52 Single Chip Microcomputer

This design is according to the requirements of the wide rider, and the Multi-function bike computer has the function of velocity measurement, display, timing, lighting, buzzer warning, which could satisfy the requirements of cycling enthusiasts. This design is based on STC89C52 SCM (Single Chip Microcomputer) smallest system and uses hall sensor receives the bicycle wheel rotation numbers date, and then transmit the received data into the SCM. After the SCM’s calculation and processing, it gets the bicycle’s instantaneous speed, mileage, riding time through the LED display the date. The part of software programming use C language, which compiles and downloads by the Keil and STC-ISP. This design uses Proteus to make the circuit diagram drawing and system simulation. The overall program adopted modular approach, and each module has a special function. The idea which makes the program more clear, is advantageous to the optimization of code logic and modification.

The Computer Crimes of Vasiliy Gorshkov and Alexey Ivanov

The purpose of this essay was to document the cybercrimes of Vasiliy Gorshkov and Alexey Ivanov, starting from their humble beginnings in Chelyabinsk, Russia to their convictions for conspiracy, violations of the Computer Fraud and Abuse Act, and other federal crimes. The paper outlines the history of these two individuals, describing the circumstances under which they were arrested and prosecuted. The essay concludes by observing that the mainstream media characterized Gorshkov and Ivanov as villainous Russian hackers, whereas in reality, they were would-be Russian entrepreneurs attempting to earn their fortune by illicit means.

The Location and Diameter of the Primary Maxillary Sinus Ostium in Malaysians: A Cone-Beam Computerized Tomography Study

Rhinosinusitis is one of widely spread diseases in the region and the role of the anatomical variations in its pathogenesis remains unresolved. A retrospective study using CBCT scan was employed to locate and measure the diameter of 320 primary maxillary ostium (PMO) (n = 160 subjects) among the Malay and Chinese populations (Mongoloid race) in Malaysia. Image analysis was performed using the i-CAT Vision Software, employing the multiplanar reconstruction window in which axial, coronal and sagittal planes were visualized in 0.3 mm intervals. The mean diameter of the PMO was significantly larger in the Chinese than the Malay. Females had larger size than the male and bilateral asymmetry was noticed, where the right side PMO was larger than the left side (p < 0.05). In addition, PMO opened more in the posterior third position of the hiatus semilunaris (61.9%) than anterior and middle third. The PMO showed a statistically significant posteriorly placed position in the Chinese than the Malays and this was more evident in the right side PMO (p < 0.01). In conclusion, the PMO commonly opens in the posterior third of the hiatus semilunaris and its diameter is significantly greater in the Chinese female with evidence of bilateral asymmetry. Awareness the anatomical variation of the Ostium diameter and location among the Malay and Chinese populations potentially has important clinical effects during surgical procedures.

Overusing the computer and internet can yield several health problems. The enhanced use of computer on can impact on both physiological and psychiatric health factors. It is accompanies with bad posture, musculoskeletal pains, obesity, weaker eyesight and mental illness. Health hazard of computer use life demands serious concern. Performing regular exercise largely can prevent or reduce the aforementioned issues that can be expected from computer and internet overuses. Physical activity such as 15 or 30 minutes walking for any break and reducing of the spending time to work with this kind of technology can account for as a helpful strategy toward living in more ideal manners.

The Pharmacist Knowledge and Computer Skills Towards E-Health. Results of A Survey among Italian Community Pharmacists

Background: Health practitioners of different disciplines (physicians, paramedic personnel, pharmacists) should possess adequate ICT (Information and Communication Technology) skills and practical competence on these technologies. Pharmacy practice requires a relevant flow of information, collection and handling of which could be facilitated by ICT. The correct use of ICT requires a proper knowledge by professionals offering a public service. The purpose of this paper was to assess basic informatics background and computer uses in pharmacists practicing in Italy, with the aim to identify educational interventions for increasing their informatics abilities. Methods: A questionnaire was proposed to pharmacists working in Italian community pharmacies. The survey was articulated into 35 questions concerning computer basic knowledge, ICT facilities, computer skills, habits, and uses in pharmacy practice. Results: The questionnaire was completed by ~45% (No. 224) of responders, the 96.9% of which used computer. Only 7.8% of interviewed people employed e-mail to communicate with health authorities and less than 2% used it to be in contact with physicians or other health professionals. Less than 40% used ICT for training courses. The results revealed a relevant percentage (90.8%) of self-learners to computer use, while only 9.2% was trained by specific courses. Conclusion: Basic knowledge of community pharmacists on ICT technological infrastructure is quite low. Although pharmacists play an essential role in health services, they have apparently limited computer skills. This suggests the need of educational and training efforts for enlarging computer and ICT resources competent use in daily pharmacy practice.  

Premenstrual Syndrome and Associated Factors among Students in Secondary Schools in Arba Minch Town, Southern Ethiopia: A Cross-Sectional Study, 2021

Background Many women feel physical or mood changes in the days before menstruation. Previous studies in other areas of Ethiopia have not looked at factors that could be related to the premenstrual syndrome. As a result, this study aims to fill these gaps by determining the prevalence and behavioral, reproductive, and menstrual factors that affect premenstrual syndrome in Arba Minch town. Methods An institution-based cross-sectional survey was conducted in Arba Minch town from April 1 to April 25, 2021. Participants were chosen using computerized simple random selection from a list of female students' registration numbers that had been transformed into unique codes. SPSS for Windows version 25.0 was used to analyze and interpret the data. Bivariate analysis was used to fit variables with a P-value of less than 0.2 for multivariable analysis. In multivariable logistic regression, variables with a P-value less than 0.05 are considered statistically significant. Result This study has revealed that 59.8% (95% CI: 55.8%–63.8%) of students have premenstrual syndrome. Dysmenorrhea (AOR = 3.172, 95% CI: 2.032, 4.952), family history of PMS (AOR = 3.155, 95% CI: 2.119, 4.697), normal body mass index (AOR = 0.244, 95% CI: 0.161, 0.37), and overweight (AOR = 3.739, 95% CI: 1.763, 7.931) were significantly associated with premenstrual syndrome. This study has revealed that 59.8% (95% CI: 55.8%–63.8%) of students have premenstrual syndrome. Dysmenorrhea (AOR = 3.172, 95% CI: 2.032, 4.952), family history of PMS (AOR = 3.155, 95% CI: 2.119, 4.697), normal body mass index (AOR = 0.244, 95% CI: 0.161, 0.37), and overweight (AOR = 3.739, 95% CI: 1.763, 7.931) were significantly associated with premenstrual syndrome. Conclusion Six out of ten Arba Minch town secondary school students had premenstrual syndrome. Dysmenorrhea, a family history of premenstrual syndrome, and body mass index were found to be significantly associated.

The trajectory of immunoglobulins immune response against the different amounts of xenobiotics matches the trajectory of biological changes associated with ageing: A systematic review

Background Ageing is a life process in which progressive molecular, cellular, physiological and anatomical changes manifesting in humans and animals including other organisms lead to the decline of biological functions. Immunoglobulins (Igs) are glycoprotein molecules produced by white blood cells mainly B lymphocytes following signal transduction as a result of their interaction with pathogenic microbes or poisonous substances introduced into the body systems. They elicit responses against the side effects of pathogens and poisons in which their response efficiency usually declines as we are ageing. Objective Thus, the similarities between Igs’ immune response against the different amounts of xenobiotics and the biological changes associated with ageing have been systematically assessed using the reports of different study results on humans and animals. Methods First, a literature search was carried out in google, PubMed and google scholar using planned search terms related to the title of this study. Review and original articles were retrieved, downloaded and saved on a computer. And then the effects of different factors i.e. xenobiotics, age, sex and lifestyle-based practices on the levels of serum Igs (IgG, IgA and IgM) in animals and humans have been studied using a systematic review of different literature sources. Finally, the relationship between the findings of various studies has been assessed and judgment on the possible cause of ageing has been made. Results The findings of different research have demonstrated that the signaling efficiency of immunoglobulin M (IgM) has been limited by the amount of test compounds administered to study Balb c mice in the oral route. The response efficiency of IgM immune response against the lower doses of test compounds were high compared to the higher doses of test compounds which was low. The results of different other studies also demonstrated that the decline of serum IgM levels was associated with ageing. The relationship between alcohol consumption and the concentration of serum Igs was also described in the report of different studies. These studies have shown that there was lower level of IgG in the blood serum of alcohol consumers compared to non-consumers. The study has also demonstrated a lower level of serum IgM with higher alcohol consumption and higher serum concentration with moderate beer consumption. Conclusion The trajectory of Igs’ immune response against different amounts of xenobiotics was highly associated with the trajectory of biological changes during ageing. These research findings might be the possible evidence to conclude that ageing is caused by the foodstuffs and non-foodstuffs we usually consume, the lifestyles we usually experience and the way of life we usually live in the environment which gradually defiling the natural processes of the body.

A review of ultrasound for the diagnosis of acute appendicitis in adults

Acute appendicitis is one of the most common surgical emergencies globally, with a lifetime incidence of 8.6% in men and 6.7% in women. While acute appendicitis should be managed promptly to reduce the morbidity associated with perforated appendicitis, morbidity from negative appendicectomy is similar to morbidity from uncomplicated appendicitis. Computer tomography is widely used to aid in the diagnosis of acute appendicitis, however, is costly, often has a slow turn around time, and is associated with exposure to ionising radiation. In contrast, ultrasound is cheap, widely available, requires minimal patient preparation, and does not require exposure to ionising radiation. Ultrasonography is becoming increasingly used for adult patients in emergency settings. The literature has estimated the sensitivity of ultrasound for acute appendicitis in adult patients as between 39-96.4%. The sensitivity and specificity of ultrasound for the diagnosis of acute appendicitis is significantly increased when the appendix is visualised. In cases of a non visualised appendix, indirect ultrasound signs can improve the sensitivity to 93.9% and specificity to 85.7%. The variation in sensitivity and specificity for ultrasound in the diagnosis of acute appendicitis in adults may be due to multiple factors. Ultrasonographer experience, a retrocaecal appendix and obesity have all been described. Given the availability, cost and potential to reduce the rate of negative appendicectomy, ultrasound should be considered as the first line imaging modality for adult patients presenting with suspected AA.

The Design and Demonstrative Analysis of the Gas Turbine Operation’s Simulator

The Gas Turbine operation was investigated with a view to evolving a system designed to provide a realistic imitation of the controls and operation of a Gas Turbine, used for training purposes. Operator Training Simulator has been widely adopted by many industries being a computer simulation which attempts to model a real-life plant so that it can be studied. A well trained and skilled operator is key in increasing power plant safety and productivity. Therefore, enabling quality training for operators is becoming more important as they need to handle increased load of information and duties whereas the lack of training is a major reason for inadequate performance. By changing variables in the simulator, predictions are made about the behaviour of the engine. It is a tool to virtually investigate the behaviour of the system while in operation. This work becomes indispensable because it is prohibitively expensive or simply too dangerous to allow trainees use the real equipment in a power plant. The Gas Turbine operation’s simulator is born from Object Oriented Programming, employing key programming languages. The simulator design focused on specific tasks in the operation of the Gas Turbine which include; startup, synchronization and monitoring of vital parameters like vibration, temperature, pressure, and angle of the Inlet Guide Vane. The statuses of various valves, pumps and motors as well as the Performance of actuators and the response of concatenated components are also being tracked. The simulator was found to effectively mimic a real plant life. With this simulator, trainee operators in Gas Turbine can spend time learning valuable lessons in a "safe" virtual environment yet living a lifelike experience. This will go a long way in minimizing operators’ error in GT power plants, thereby curtailing power outages and conserving power plant components.

Users Perception and Factors Affecting Data Quality in Nyarugenge Public Health Facility, Rwanda

The study evaluated the users' opinions on data quality and related characteristics in ten public-health institutions of Nyarugenge district. A study used a cross-sectional design, data was collected through quantitative (n=150) methodology while qualitative data was obtained using interviews (n=20) and focus group talks (n=3). A checklist was utilized to examine the completeness, accuracy, and timeliness of data quality aspects. Collected quantitative data was analyzed through logistic regression by SPSS to examine the association of variables, while qualitative data was analyzed using the summative content analysis (SCA) to summarize the key themes. A 95 percent confidence level, Odds Ratio (AOR) were used to establish the strength of correlation among study variables, while a p-value of less than (p<0.05) was utilized to identify the variables which were statistically significant associated to HMIS data quality. The study finding showed that the majority (53.0%) was female while, 33 years was an average age, the majority of health practitioner (52.7%) had an A1 diploma. Approximately 90.6% of respondents have a positive perceptions on the system usage. Limited ability and a lack of relevant technology equipment such as computers and the internet have been cited as the challenges while using the system. Factors such as training AOR:2.62(95% CI:1.45, supervision AOR:1.81(95% CI:1.02, AOR:2.50(95% CI:0.85 for education background, works-experience AOR:1.60(95% CI:090 are factors associated with data quality. Maintaining, supportive supervision, regular training and refresher courses should be regularly offered to public health professionals to improve their knowledge in order to maximize the use of health information.

Fingerprints are commonly used for identification 1. All records are computerized. In a catastrophe recorded images of soft tissue may become useless if the tissues burn throughout. Dental prints are rarely used for identification. Information is taken from dental radiographs or dental casts. In case of fire recorded images of dental structures may be useful because they do not readily burn.

The Battle for Backdoors and Encryption Keys

This paper argues that the use of backdoors in software is inherently counterproductive and leads to invasion of privacy, either by federal or state governments or by intrusive hackers. The essay outlines encryption’s nature, pointing out that a software backdoor is a secret means of ignoring data authentication. Several examples of known backdoors known to terrorists, criminals, and governments alike are highlighted. Arguments in favor and opposing backdoors are provided, where the Apple Computer, Inc. v. FBI controversy is discussed. Finally, the balancing of harms test as proposed by John Stuart Mill is introduced, where the article concludes that when balancing the opposing positions, the scale tips toward data encryption because an innocent party would suffer the most harm from the existence of a software backdoor.

Adjacent Segment Disease Associated with Klippel-Feil Syndrome: A New Classification System with Corresponding Therapeutic Options

Background Klippel-Feil Syndrome (KFS) is described as the clinical triad of short neck, low posterior hairline, and limitation of neck mobility. Objective Topresent our clinical experience with KFS patients who also had adjacent segment disease (ASD) and to propose a novel classification system for these patients. Methods This retrospective study was performed in the neurosurgery department of our tertiary care center. Data were gathered using the medical records of 22 KFS patients (10 males, 12 females) with ASD. Diagnosis was confirmed with imaging modalities including X-ray, computerized tomography, and magnetic resonance imaging. Clinical and radiological findings as well as therapeutic outcomes were noted. Results The average age of our series was 56.09 (range: 41 to 67) years. The operative technique was selected as for our novel “Yilmaz-Yucesoy Classification System”. Accordingly, one patient (4.54 %) received non-surgical treatment (Yilmaz-Yucesoy Grade 1), six cases (27.27 %) underwent anterior cervical arthroplasty, seven patients (31.81 %) underwent anterior cervical discectomy or corpectomy and fusion with cervical cage and plate (Yilmaz-Yucesoy Grade 3). Eight patients (36.36 %) with cervical spinal instability had anterior cervical discectomy or corpectomy and fusion with cervical cage and plate (Yilmaz-Yucesoy Grade 4). No mortality or remarkable complications were detected. Conclusion Appropriate and timely recognition and classification of patients with KFS and ASD based on our newly proposed “Yilmaz-Yucesoy Classification System” yielded promising treatment outcomes. However, further prospective, randomized, controlled trials are warranted on larger series to validate our preliminary results.  

Digital Planning for Mini-Implant Supported Palatal Expander in Open-Bite Treatment

Open bite malocclusion, due to it’s multifactorial etiology, has always been considered a difficult problem to treat. Often associated with transverse maxillary deficiency, this is a real challenge in the field of orthodontics. The traditional approach, for this type of anomaly, in adult patients, is orthognathic surgery and RME (rapid maxillary expansion). There are several approaches to the treatment of adult patients using digital technology. Mini-implant supported palatal expander limits the side effects of the conventional RME and is less invasive compared to orthognathic surgery. Precise and predictable mini-implant insertion, using a customized surgical guide, provides a safe therapeutic approach. This case report combines Cone-beam computed tomography (CBCT), laser scan superimposition, computer-aided design (CAD) and 3D printing in order to design and print a customized surgical guide for orthodontic mini-implant insertion. A CBCT scan was performed to determine the optimal site for mini-implants’ placement. Using the 3Shape Trios Intraoral Scanner the maxilla and the mandible were laser-scanned. Blue Sky Plan 4 software was used to design the surgical guide, and RayWare software was used for printing it. 4 mini-implants were inserted using a safe and predictable technique. The 3D technology represents the future of orthodontics, reducing the risks, chair-side time while providing the best treatment plan for the patient.

GATES/GEB as the Best Thermodynamic Approach to Electrolytic Redox Systems - a Review

The Generalized Approach To Electrolytic Systems (GATES) provides the best possible thermodynamic formulation of redox and non-redox, equilibrium and metastable, mono-, two- and three-phase systems, with all attainable/pre-selected physicochemical knowledge involved, without any simplifying assumptions made for calculation purposes, where different species may occur in batch or dynamic systems, of any degree of complexity. The Generalized Electron Balance (GEB) is the key concept completing the set of algebraic balances referred to redox systems, described according to GATES/GEB ⊂ GATES principles. The GEB, considered as the law of Nature, is fully compatible with charge and concentration balances, and relations for the corresponding equilibrium constants. Within GATES, the electrolytic systems are resolvable with use of MATLAB, or other iterative computer programs, if all necessary physicochemical knowledge is attainable.

Closed Frequent Itemsets Mining Based on It-Tree

In the last decade, the amount of collected data, in various computer science applications, has grown considerably. These large volumes of data need to be analysed in order to extract useful hidden knowledge. This work focuses on association rule extraction. This technique is one of the most popular in data mining. Nevertheless, the number of extracted association rules is often very high, and many of them are redundant. In this paper, we propose an algorithm, for mining closed itemsets, with the construction of an it-tree. This algorithm is compared with the DCI (direct counting & intersect) algorithm based on min support and computing time. CHARM is not memery-efficient. It needs to store all closed itemsets in the memory. The lower min-sup is, the more frequent closed itemsets there are so that the amounts of memory used by CHARM are increasing.

Using a Medication Plan as a Quality Indicator: Feasibility and Satisfaction Results from an Observational Study

Background Medication adherence remains a challenge for patient management. Changes in the drug regimen after a hospital stay can lead to confusion or misunderstandings. We implemented a structured patient-centered interview during which a computer-generated individualized medication plan was discussed and provided to patients at discharge. Objective To explore whether a medication plan can be a quality indicator, in terms of its content (quality) and its implementation in the resident’s workflow (feasibility). Methods An observational mixed method study with interviews of 174 patients from general internal medicine wards at 1 week and 1 month after discharge, and of 91 physicians at baseline. We report the quality of the medication plan in terms of content and state of completion. We describe feasibility for residents to complete this plan, as well as patient and resident satisfaction with the plan. Results 83% of participants received a medication plan. Physicians verified renal function (83%) to adapt doses but did not regularly assess for medication interactions (43%). Incomplete plans (61%), were due to blanks when physicians considered the information irrelevant for their patients. Error rate was <3%. Patients reported low use of their plan after discharge (64% found it useful after 1 week, whereas only 37% used it when taking their medication 1 week after discharge). Conclusion Although the plans were considered useful by both patients and physicians, their implementation could have been optimized by considering the overall process (creation to patient use). Mobile apps could help fill gaps in supporting patients for medication adherence.

The quantum model of electric current suggested by Feynman has been enlarged by n difference-differential Hamiltonian equations describing the phonon dynamics in one dimensional crystallyne lattice. The process of interaction between the electron and phonon components in a crystalline lattice of a conductor has been described by 2n parametrically coupled difference-differential Hamiltonian equations. Computer analysis of the system of these coupled equations showed that their solutions represent a form of the quantum recurrence similar to the Fermi-Pasta-Ulam recurrence. The results of the research might reconsider the existing concept of electric current and will be possibly helpful in developing an acoustic «laser».

Does Soil Testing for Fertiliser Recommendation Fall Short of a Soil Health Card?

Nutrient depletion and imbalanced use of fertiliser nutrients, inappropriate tillage and rain- water management practices often result in land degradation. Declining soil health contributes to climate change through loss in soil productivity, biodiversity, soil carbon, and moisture and ecosystem services. In order to address declining soil health, government of India has launched a soil health card (SHC) scheme aimed at need base use of chemical fertilisers. The paper points out the short-comings in the SHC scheme. Balanced and need base use of chemical fertilizers can be helpful in environmental protection and restoring soil health. The paper identifies potential agronomic practices and production management systems that can reduce our dependence on synthetic nutrients. Integration of soil fertility management domains with computer based QUEFT crop model has the potential of making fertiliser recommendations more domain and crop specific and less cumbersome. For soil health assessment chemical indicators must be integrated with physical and biological properties of the soils which can be predicted through reflectance spectroscopy. For assessing soil health related issues across different agro-ecoregions, there is however an urgent need for building-up more robust soil reflectance libraries.  

Prevalence of Diarrhea and Associated Factors among Under Five Years Children in Harena Buluk Woreda Oromia Region, South East Ethiopia, 2018

Background: Acute diarrheal diseases are the leading cause of preventable childhood death, especially in developing countries. It is the second leading cause of death in under-five year children nest to pneumonia. Objectives: The aim of this study was to determine the magnitude and associated factors of diarrhea in under-five in Harena Buluk district. Methods: A community based Cross sectional study was conducted in February, 2018. A two stage stratified sampling method was done to select the eligible households. Data was collected by trained data collectors using pretested questionnaire list which was prepared based on EDHS and WHO core questionnaires related to diarrhea. Data was entered in to a computer using Epi data 3.1 and exported to SPSS V.20 for further analysis. Logistic regression was used to determine level of association with 95% CI. A p-value <0.05 in the final model were considered as significant. Result: The two weeks period magnitude of diarrhea among under-five children was 28.4% with 95% CI (14.5-20.8) which was associated with households with one under five children (AOR: 0.268, 95% CI(.08,0.90)), living in the home with single room ((AOR = 6.01, 95% CI(1.01,36.01)), clean latrine/faces not seen around the pit or on the floor of latrine(AOR: 0.298, 95% CI(0.097,0.92)), long time take to fetch water from source (AOR: 0.046,95%CI(0.01,0.22)), home based water treatment ((AOR = 0.15, 95% CI: (0.04, 0.62)), living with animal in the same house(AOR: 8.31, 95% CI(2.46,28.06)), children who took gruel type of food(AOR: 0.24, 95% CI(0.07,0.81)), hand washing practice before cooking food (AOR: 0.195(0.066, 0.574)). Conclusion: The two weeks period magnitude of acute under-five diarrhea was relatively high and number of under five children in the household, cleanness latrine, time taking from the source of water, home based water treatment, number of rooms in the home, live with animal in the same house, type of food child take, hand washing before food preparation had significant association with the occurrence of under-five diarrhea. Improve community about home and environmental sanitation and hand practices were recommended.

Towards Implementing the Integrated Technology of Precision Agriculture in Sudan

This study reviews the agricultural development perspective in the light of a rapid space technology development. In other words, precision agriculture as part of geoinformatics. The aim is to quantify whether the adopted technology can improve the efficiency of agricultural fields management and production to attain food security. Therefore, views of targeted groups from different States of Sudan were investigated, using stratified sampling method. Where quantitative statistics (descriptive/inductive techniques) was applied. About 800 questionnaires were distributed. The outcomes of data analysis reflected that the majority of interviewed groups 357 (82.1%) do not know the principles and application of integrated technology in the field of agricultural management. 85.3% of respondents know nothing about computer program related to precision agriculture. The majority of the respondents (84.6 %), did not get courses on precision agriculture during the under graduate study. The result also revealed that only 11.8% of the respondents use modern techniques in land preparation, 16.1 % in soil analysis, 12.5 % in the field of seed technology, and 11.4% in crop harvesting. However, 53.9% of the respondents reported that their Departments did not care about training on agricultural precision. Nevertheless, 24.3 % of the respondents got trained on precision agriculture through personal efforts, while about 19% got trained by their respective Departments. In regard to education, 16% of the respondents got trained on precision agriculture at undergraduate and only 9% after graduation. The study concludes that despite the rapid technological development, agriculture in Sudan remained lagging, and the productivity is below the expectation. It recommends that the Ministries of Agriculture in different States in Sudan should take the issue of introduction of new technology seriously to boost the agricultural development to attain food security.

Title - Case of Unilateral Congenital Aural Atresia & Microtia with Cholesteatoma

We report a case of cholesteatoma associated with unilateral congenital aural atresia (CAA) and microtia, resulting in extracranial complication. A 13-year-old girl presented with right ear purulent discharge from pre and post auricular area with mastoid tenderness. On clinical examination, she had right grade II microtia with pre-auricular skin tag and healed scar tissue in periauricular region. On High Resolution Computerized Tomography (HRCT) scan of temporal bones, there was CAA of right ear with extensive cholesteatoma in middle ear cleft. The cholesteatoma was subsequently removed by radical mastoidectomy with uneventful post-operative period. Cholesteatoma associated with CAA may remain undiagnosed in early childhood resulting in life threatening temporal and intracranial complications.

Medical Management of Sexual Assault Survivors at an Academic Medical Center

Sexual assault is a serious problem in the United States. It is crucial that the medical management of these patients is adequate. The Centers for Disease Control and Prevention (CDC) provides guidelines for appropriate management of these patients. The purpose of this study is to evaluate medical management of survivors of sexual assault at an academic medical center and identify opportunities for improvements in care. We conducted a retrospective, chart review study of patients aged 12 years and older presenting at an academic medical center following sexual assault or rape between 2009 and 2013. Descriptive statistical tests were used to analyze the data. A total of 29 females and 5 males were identified. The majority of patients presented to the medical center within 24 hours of sexual assault or rape and typically presented to the emergency department. Empiric treatment for infections were 8.8% for Hepatitis B, 29.4% for human immunodeficiency virus, 20.6% for gonorrhea, 17.6% for chlamydia, and 8.8% for trichomonas. Among women of reproductive age, 28.0% were provided with emergency contraception. This study found inconsistency in the medical management of sexual assault survivors. Among this small sample size, many patients were not provided with recommended medical treatments. With this knowledge, the medical center plans to implement institutional guidelines and a corresponding order set in the computerized prescriber order entry system to standardize the medical management of sexual assault survivors and educate healthcare professionals. Future studies are warranted to evaluate the impact of standardized guidelines and order set implementation.

Efficacy of Calcium Phosphosilicate (CPS) Putty As Alloplastic Bioactive Graft Material in Sinus Augmentation Procedures: An Original Study

Background and Context: Autologous bone grafts require a second surgical exposure to harvest the graft with a significant risk of post-operative complications and donor site morbidity. Employment of allografts potentially eliminates donor site morbidity but carries the potential of causing foreign body immune reactions with a high rate of rejection and failures. Hence, Alloplasts have emerged as novel materials to overcome the drawbacks of autogenous and allogenous bone grafts. Today’s advanced dentistry is enterprising a putty form of calcium phosphosilicate (CPS) into several aspects of reconstructive domain of dentistry including the sinus augmentation procedures, osseous regeneration of periodontal bone defects, cystic cavity defects and alveolar socket preservation. This eccentric multifaceted study was planned as an honest effort on the path of proving the efficiency of CPS as an alloplastic bioactive graft material and aimed at quantitative evaluation of regenerated bone radiographically using Cone Beam Computed Tomography (CBCT) scans following sinus augmentation procedures. Materials and Methods: A total of 20 sinus augmentations were carried-out in 17 patients satisfying the criteria (inclusion and exclusion) requiring placement of implants in atrophic maxilla and in other situations of anatomic constraints. The graft material used was calcium phosphosilicate (CPS) putty (Nova bone dental putty, Novabone products, Alachua, Fla). The residual bone height was recorded using CBCT scans and a computer based software where the measurements were made from the crest of the ridge till the sinus floor lining. The patients were assessed clinically at immediate post-operative, 1 week, 1 month, 3 months and 6 months follow-up recalls post-operatively. Radiographic assessment for bone height was done pre-operatively and at 6 months follow-up recall post-operatively using Cone Beam Computer Tomography scans. Results: The post-treatment bone height (11.76+0.97mm) was found to be significantly higher than the pre-treatment bone height (6.8 +0.70mm) in indirect sinus augmentation procedures (t=14.4, p<0.005). In case of direct sinus augmentation group also, the post-treatment bone height (11.27 +0.71mm) came-out to be significantly higher than the pre-treatment bone height (2.44 +0.81mm) (t=32.17, p<0.005). The post-treatment bone grafted sinus floor measurements (1107.6 + 155.6mm) were also found to be significantly higher than the original sinus floor measurements of bone density (Grayscale values) (412+ 65.5mm) (t=16.6, p<0.001) in case of indirect sinus augmentation procedures. Similarly, the post-treatment bone grafted sinus floor measurements (1169.6+136.7mm) were found to be significantly higher than the original sinus floor measurements of bone density (Grayscale values) (416.4+ 0.70mm) (t=17.9, p<0.001) in case of direct sinus augmentation group. Conclusion: Calcium Phosphosilicate (CPS) was accepted well at the recipient sites without any complications demonstrating its efficiency and reliability in sinus augmentation procedures.

Anatomical Risk Factors Associated with Immediate Extraction Placement in the Posterior Maxilla: A Human Retrospective Cone-Beam Study

Purpose: The aim of the study was to analyze the spatial relationship between the sinus floor and the alveolar bone of maxillary premolars and molars in order to assess the amount of basal bone available for immediate extraction placement of endosseous implants. Materials and Methods: All the cone-beam computerized tomographic (CBCT) scans taken over a three-year period at four centers were identified. The subsinus bone height (SBH) was evaluated by measuring the distance between the sinus floor and the first and second premolar root apexes. The interradicular septum bone height (ISBH) of all the first and second molars was also measured, and the presence or absence of sinus-floor invagination between the molar roots was recorded. Results: Five hundred twenty-six (526) CBCT scans of fully or partially dentate maxillary arches were examined. The root apex was inside the sinus of 1.8% of all the first premolars. It approximated the sinus floor of 5.3% of them, and the distance between the two points was between 1 and 3mm of 20%, between 3 and 7mm for 34.1%, and more than 7mm for 34.1%. For the second premolars, the findings were 13.5% (inside the sinus), 10.5% (touching the sinus), 32.8% (1-3mm of distance) 28% (3-7 mm), and 15.2% (more than 7mm). Root invagination was noted in 50.2% of the first molars and 43.1% of the second molars, with the ISBHs ranging from 13.4% to 56.9%. Conclusion: Radiological evaluation is essential to determining whether implants should be placed immediately after extraction of maxillary premolars and molars.

Computational STAT4 rSNP Analysis, Transcriptional Factor Binding Sites and Disease

Purpose Signal Transducer and Activator of Transcription 4 (STAT4) is important for signaling by interleukins (IL-12 and IL-23) and type 1 interferons and has been found to have several simple nucleotide polymorphisms (SNPs) associated with human disease. STAT4 SNPs were computationally examined with respect to changes in potential transcriptional factor binding sites (TFBS) and these changes were discussed in relation to human disease. Methods The JASPAR CORE and ConSite databases were instrumental in identifying the TFBS. The Vector NTI Advance 11.5 computer program was employed in locating all theTFBS in theSTAT4 gene from 4 kb upstream of the transcriptional start site to 8.3 kb past the 3’UTR. The JASPAR CORE database was also involved in computing each nucleotide occurrence (%) within the TFBS. Results The STAT4 SNPs in the 70 kb intron between exon 2 and 3 are in linkage disequilibrium and have previously been found to be significantly associated with several vasculitis diseases as well as diabetes. The SNP alleles were found to alter the DNA landscape for potential transcriptional factors (TFs) to attach resulting in changes in TFBS and thereby, alter which transcriptional factors potentially regulate the STAT4 gene. These STAT4 SNPs should be considered as regulatory (r) SNPs. Conclusion The alleles of each rSNP were found to generate unique TFBS resulting in potential changes in TF STAT4 regulation. These regulatory changes were discussed with respect to changes in human health that result in disease.

Computational EPAS1 rSNP Analysis, Transcriptional Factor Binding Sites and High Altitude Sickness or Adaptation

Purpose The endothetal Per-Arnt-Sim (PAS) domain protein 1 (EPAS1) gene which encodes hypoxia-inducible-factor-2 alpha (HIF2a) is a transcription factor that is involved in the response to hypoxia. EPAS1 has been found to have four (rs56721780, rs6756667, rs7589621, rs1868092) simple nucleotide polymorphisms (SNPs) associated with human disease.These SNPs were computationally examined with respect to changes in potential transcriptional factor binding sites (TFBS) and these changes were discussed in relation to disease and alterations in high altitude adaptation in humans. Methods The JASPAR CORE and ConSite databases were instrumental in identifying the TFBS. The Vector NTI Advance 11.5 computer program was employed in locating all theTFBS in theEPAS1 gene from 1.6 kb upstream of the transcriptional start site to 539 bps past the 3’UTR. The JASPAR CORE database was also involved in computing each nucleotide occurrence (%) within the TFBS. Results The EPAS1 SNPs in the promoter, intron two and the 3’UTR regions have previously been found to be significantly associated with disease and different levels of high-altitude hypoxia among native Tibetans. The SNP alleles were found to alter the DNA landscape for potential transcriptional factors (TFs) to attach resulting in changes in TFBS and thereby, alter which transcriptional factors potentially regulate the EPAS1 genesuch as for the glucocorticoid and mineralocorticoid nuclear receptor binding sites created by the rs7589621 rSNP EPAS1-G allele. These receptors regulate carbohydrate, protein and fat metabolism. Also the minor rs7589621 rSNP EPAS1-A creates a punitive TFBS for the FOXC TF which is an important regulator of cell viability and resistance to oxidative stress. These EPAS1 SNPs should be considered as regulatory (r) SNPs. Conclusion The alleles of each rSNP were found to generate unique TFBS resulting in potential changes in TF EPAS1 regulation. The punitive changes in TFBS created by the four rSNPs could very well influence the significant cline in allele frequencies seen in Tibetans with increasing altitude or the haplotype association with high altitude polycythemia in male Han Chinese. These regulatory changes were discussed with respect to changes in human health that result in disease and sickness.
