Identity Crisis - Common Tumors in Exceptional Locations, A Case Series

Mucoepidermoid carcinoma (MEC) accounts for only 5% of all salivary gland tumors and is most often seen in the parotid glands. MEC occurrence in the larynx is, however, rare. The incidence of primary squamous cell carcinoma (SCC) of salivary glands is also scarce and comprises only about 1.6% of all salivary gland malignancies. Hereby, we share our experience in managing two patients with rare and opposite variants of malignancy which were diagnosed at the same time; MEC of the larynx and SCC of the parotid. In MEC tumors, the presence of the intermediate and mucous cells with positivity in mucicarmine stain are the significant features. For SCC tumors, identification of the usual tumor markers (p40, CK 5/6 and p63) are pathognomonic. Although MEC and SCC are common in the head and neck regions, the existence of these malignancies in exceptional locations must be considered. The key features mentioned in our comparison table can help distinguish both these tumors and to deliver the correct treatment modalities. The prevalence of genomic and carcinogenic factors in the occurrence of these tumors in uncommon locations needs to be explored in future studies.

The Location and Diameter of the Primary Maxillary Sinus Ostium in Malaysians: A Cone-Beam Computerized Tomography Study

Rhinosinusitis is one of widely spread diseases in the region and the role of the anatomical variations in its pathogenesis remains unresolved. A retrospective study using CBCT scan was employed to locate and measure the diameter of 320 primary maxillary ostium (PMO) (n = 160 subjects) among the Malay and Chinese populations (Mongoloid race) in Malaysia. Image analysis was performed using the i-CAT Vision Software, employing the multiplanar reconstruction window in which axial, coronal and sagittal planes were visualized in 0.3 mm intervals. The mean diameter of the PMO was significantly larger in the Chinese than the Malay. Females had larger size than the male and bilateral asymmetry was noticed, where the right side PMO was larger than the left side (p < 0.05). In addition, PMO opened more in the posterior third position of the hiatus semilunaris (61.9%) than anterior and middle third. The PMO showed a statistically significant posteriorly placed position in the Chinese than the Malays and this was more evident in the right side PMO (p < 0.01). In conclusion, the PMO commonly opens in the posterior third of the hiatus semilunaris and its diameter is significantly greater in the Chinese female with evidence of bilateral asymmetry. Awareness the anatomical variation of the Ostium diameter and location among the Malay and Chinese populations potentially has important clinical effects during surgical procedures.

An Inherited Balanced Translocation Between Chromosomes 4 and 6 in Recurrent Pregnancy Loss: A Case Report

Recurrent pregnancy loss is an important reproductive issue with a heterogeneous etiology where two or more consecutive abortions occur before 20 weeks of gestation. Approximately 15% of all clinically recognized pregnancies result in miscarriage with an incidence of 1 in 300 cases. Couples, who experience repeated pregnancy loss before three months of gestation, mostly have fetuses with chromosomal aneuploids. A non-consanguineous couple with a married life of 4 years was referred to the Institute with a clinical history of three first trimester abortions. Karyotype analysis revealed a balanced autosomal translocation between chromosomes 4 and 6 with 46, XX, t (4;6)(q35; q22) karyotype in the female and normal 46, XY in the male partner. Therefore, the siblings and the couple were suggested for extended genetic counseling. Interestingly, similar translocation was seen in her father and three sisters, whereas her mother and elder sister showed a normal chromosomal constitution, indicating the paternal inheritance.

Non-Hodgkin Lymphoma of External Auditory Canal – Uncommon Location and Related Risk Factors

Objective: Presentation of rare case report of primary non-Hodgkin Lymphoma of temporal bone secondary to immunodeficiency after prolonged steroid administration in a diabetic patient. Then review the literature of similar case reports. Method: 49 years old female presented with hearing loss, facial paralysis and otalgia. Biopsy revealed high grade non Hodgkin’s B-lymphoma. She underwent chemotherapy and had remarkable response. Result: Rapidly progressive deafness, facial paralysis and otalgia are common presentations of external canal Non-Hodgkin lymphoma. The disease is common in middle age, females and has a fair response to chemotherapy, good to radiotherapy. Surgery is an alternative method in isolated lesions or recurrent cases. Conclusion: High dose steroid and type II diabetes are potential risks for acquired immunodeficiency. External Auditory canal lymphoma is uncommon sequel. Chemotherapy is very effective in treatment of isolated cases without B symptoms

A Case of Pernio Associated with Cold Agglutinin Positivity in an Unusual Location

Pernio is an inflammatory disease that is usually characterized by the development of cold-induced erythrocyanotic skin lesions located in acral regions. Diagnosis is difficult if lesions are present in rare locations. A 21-year-old female suffering from fatigue presented with a 4-day history of painful, purple lesions on both thighs. She worked as a cashier, wore tight clothing, and was otherwise healthy except for mild obesity. Dermatological examination revealed tender and painful edematous papules and plaques on the outer aspects of her thighs. The patient was diagnosed with pernio based on her medical history, and clinical and histological findings. Laboratory tests were normal except for her cold agglutinin titer (1:4). Pernio rarely occurs on the thigh, except in people who ride horses. However, it sometimes occurs in non-riders depending on lifestyle and environmental conditions. Therefore, lesions in this region must be confirmed by a thorough medical history and evaluation of clinical and histopathological findings. Cold agglutinins may be associated with this disease.

Random Forest Classifier for Respiratory Mortality Analytics

Respiratory diseases remain a major contributor to hospital morbidity and mortality worldwide, particularly among elderly patients and individuals with severe pulmonary compromise. Accurate prediction of respiratory mortality is clinically important for triage, resource allocation, ICU utilization, and early intervention. Traditional statistical models frequently demonstrate limited predictive sensitivity because respiratory mortality is influenced by complex interactions among demographic, diagnostic, physiologic, and severity-related variables. In this study, a machine learning framework was developed to predict in-hospital mortality among patients with respiratory disease using administrative and clinically derived variables, including age, sex, length of stay (LOS), diagnostic descriptions, risk of mortality and severity scores. A Random Forest classifier with balanced class weighting was developed and implemented to address nonlinear relationships and class imbalance within the dataset. Initial modeling demonstrated good overall discrimination performance, with receiver operating characteristic area under the curve (ROC-AUC) values approaching 0.84; however, mortality recall remained limited because deceased patients represented a minority class within the original dataset. To improve mortality detection, a physiologically informed synthetic augmentation strategy was developed. Synthetic clinical variables included oxygen saturation, ICU status, ventilator support, sepsis status, systolic blood pressure, creatinine, and lactate levels. Conditional physiologic consistency rules were incorporated during augmentation to preserve clinically plausible relationships among respiratory failure, hemodynamic instability, and organ dysfunction. The augmented dataset substantially improved model sensitivity and balanced mortality classification performance. Final model evaluation demonstrated strong predictive capability, achieving approximately 97% classification accuracy with balanced precision and recall across mortality classes. Confusion matrix analysis revealed marked reduction in false-negative mortality predictions compared with baseline modeling approaches. Feature importance analysis identified physiologic instability markers, respiratory severity classifications, LOS, and diagnostic respiratory categories as dominant predictors of mortality. These findings suggest that hybrid simulation-augmented machine learning frameworks may provide a valuable strategy for respiratory mortality analytics, particularly in datasets with limited real-world mortality prevalence and incomplete physiologic representation.

Mathematical Modelling of Typhoid Fever Transmission Dynamics and Intervention Impact in Harare, Zimbabwe (2018–2020)

Background Typhoid fever remains a significant public health issue in Harare City, Zimbabwe, exacerbated by recurrent outbreaks between 2018 and 2020. Key challenges, including inadequate water supply and sanitation infrastructure, high population density, and limited healthcare access, have intensified the disease burden. Understanding the key transmission drivers and assessing the impact of various interventions are essential for informing policy and health strategies. Objectives This study aimed to: 1: To predict future trends in typhoid fever cases Harare City typhoid hot areas. 2: To develop a mathematical model to simulate the spread of typhoid fever incidence under different intervention scenarios and recommend evidence-based strategies for reducing the disease burden in Harare City. Methods A dynamic compartmental SIR-based model, adapted from the Pitzer Vaccine Effectiveness (VE) framework, was employed to simulate disease transmission. This model accounted for both short-cycle (human-to-human) and long-cycle (environmental) transmission pathways. Data from Harare City (2018–2020) were used for model calibration and forecasting, and sensitivity analysis was performed to assess the impact of different intervention levels. Findings The model identified inadequate sanitation, contaminated water sources, and low health- seeking behaviors as primary drivers of typhoid transmission. In the absence of interventions, the model projected a sustained high rate of transmission. However, treatment and WASH interventions could reduce the disease burden by 50–60%, while combined strategies incorporating vaccination and education led to an 80% reduction in cases. Sensitivity analysis indicated that treatment and WASH interventions were particularly impactful at moderate coverage levels. Conclusion Mathematical modeling effectively demonstrated the multifactorial drivers of typhoid fever transmission in Harare. Integrated interventions that combine WASH, vaccination, treatment, and education present the most promising approach for long-term control of the disease. The findings offer a solid, data-driven foundation for public health decision-making and resource allocation.

Monitoring of Insect Species Richness and Abundance in Sudan Semi-arid Ecosystem (Case study: Khartoum State/Sudan)

Sudan's ecological landscape is characterized by various zones, including deserts, semi-deserts, and savannas, which host a diverse array of forests, rangelands, and numerous plant and animal species. However, the sustainable management of these resources is becoming increasingly challenging due to human activities and the impacts of climate change. This article presents the results of a research project focused on monitoring and documenting biodiversity in Sudan's semi-arid regions, particularly emphasizing insect diversity. Insects are vital for maintaining ecosystem health through processes such as nutrient cycling, decomposition, pollination, and acting as natural predators to pests. Field surveys were conducted across three Sudanese states—North Kordofan, Gezira, and Khartoum—to assess the diversity of flora and fauna. Specifically, El Rawakeeb semi-arid land in Khartoum state was investigated, which comprised three sites with varying soil elevations: a hilly site at 420 meters above sea level, a flat site at 416 meters, and a valley site at 411 meters. Researchers utilized direct collection methods and butterfly nets to gather insect specimens, which were subsequently preserved and identified for further analysis. Diversity indices, including Simpson’s Diversity Index, Shannon-Wiener Index, and Margalef Richness Index, were computed to compare the biodiversity of these sites, along with metrics for evenness and species richness. Permanova analysis was carried out for insect variation between the study sites. A total of 1541 insect specimens were gathered from El Rawakeeb, encompassing 34 species distributed across 21 families and 9 orders. The order Lepidoptera exhibited the greatest number of species (35.29%), followed by Coleoptera (20.59%). The orders Neuroptera, Orthoptera and Thysanoptera recorded the fewest species (2.94% each). From the hilly site, 514 insect specimens were collected, representing 9 orders, among which Hymenoptera included 4 species, while Lepidoptera had 3 species, and Odonata, Orthoptera, and Thysanoptera each had 1 species. The highest individual counts were recorded for Copriniklobe (dung beetle), Dacus longistylus (fruit fly), Camponotussp. (carpenter ant), and Anax ephippiger (dragonfly). In a flat site, 484 insect specimens were collected, classified into 15 species, 12 families, and 7 orders. The highest individual counts were noted in four species: Onthophagusnuba (dung beetle), Dacus longistylus (fruit fly), Camponotussp. (carpenter ant), and Anax ephippiger (dragonfly). In a valley site, 543 insect specimens were collected, categorized into 17 species, 16 families, and 9 orders. Insect Diversity in three research sites. Simpson's Diversity Index revealed varying levels of insect diversity across the three sites. The valley site exhibited the highest diversity level with an index score of 0.857, followed by the flat site at 0.826, and the hilly site at 0.819. The Shannon diversity index of 2.22 indicated the highest diversity among the three sites, suggesting a relatively diverse insect community. The relative abundance of insects was greatest in the valley site at 35.24%, followed by the hilly site at 33.35%, and the flat site at 31.41%. The Margalef Richness Index indicated a moderate level of richness in the insects collected from the three locations. Certain insect species were found in some sites but were absent in others. The PERMANOVA revealed notable variations among the three study sites (F = 2.24, p-value = 0.035).

Systematic Review of Spinal Cord Injuries in Equestrian Athletes: Incidence, Risk Factors, and Outcomes

Objective The goal of this systematic review is to identify common themes amongst acute spinal cord injuries (SCI) in equestrian athletes. Design A systematic review was performed using PubMed, CINAHL Plus with Full Text (EBSCO), Cochrane Library, and Scopus with pre-determined MESH terms. The initial search returned 354 studies. Following PRISMA guidelines, 13 articles were included. Exclusion criteria included injuries to the horse only, non-English language, cauda equina, and case reports. Data extraction was completed, and common findings were evaluated narratively due to heterogeneity of data. Results Seven manuscripts listed specific horse-related activities that caused SCI, with fall from horse as the highest percentage of injury. Nine articles identified the injury region, with large variations and no clear dominant area of injury. Five articles identified the length of hospital stay with ranges from 1 to 82 days. Four articles looked at the association of professional vs non-professional riders. Only two articles evaluated helmet use at time of injury, with one article showing 81% of those with SCI used helmets, and the other showing only 35.6% utilized this safety measure. Conclusion SCI in equestrian athletes can have a wide presentation, with large variation on location of injury, length of stay, and other factors. However, non-professional riders are at greater risk of SCI and individuals are more likely to sustain injury from a fall from a horse rather than a kick or another modality of injury. Future study can elicit presenting symptoms, types of surgical intervention used, and long-term outcomes and recovery.

Traumatic Maculopathy (Berlin´S Edema) Treated with Enhancement of Human Photosynthesis®

Commotio retinae (CR) is a condition frequently observed in clinical practice, particularly following closed globe trauma (CGT) due to sport, labor, or traffic accident injuries. It is the main cause of unilateral vision loss in male patients aged between. It is characterized by transient gray-white retinal coloration and reduction of visual acuity (VA). Symptoms depend mainly on the location and severity (deep) of the injury, with less complains when only the superficial or peripheral retina is affected. It may be confined to the posterior pole, when it is also called Berlin’s edema, after the first hypothesis of Berlin (1873). There is no specific treatment since the treatment depends on the region of the retina and choroid affected. In this work we report a case of Berlin edema, treated with ǪIAPI 1®, to restore the balance of oxygen, which is generated at the intracellular level.

Standardizing Use of Mepiquat Chloride for Higher Productivity of Densely Planted Bt Cotton Variety

Standardizing use of mepiquat chloride for higher productivity of densely planted Bt cotton variety” was carried out at the Regional Research Station, Abohar and Regional Research Station, Faridkot during the kharif season of 2021 on Btcotton variety PAU Bt3. The soil of experimental field was silty loam at Abohar and sandy loam at Faridkot. The experiment was laid out in a split plot design with four plant spacings (67.5×15 cm, 67.5×30 cm, 67.5×45 cm and 67.5×60 cm) as main plots and five Mepiquat chloride levels (MC (5%) @ 375 ml ha-1 at squaring, MC (5%) @ 375 ml ha-1 at flowering, MC (5%) @ 750 ml ha-1 at squaring, MC (5%) @ 750 ml ha-1 at flowering and after 15 days and Control (No MC)) as sub plots with three replications. Lowest plant density (67.5×60 cm) and MC (5%) @ 750 ml ha-1 at squaring resulted in significant reduction of plant height, LAI (Leaf Area Index) and dry matter as compared to control at both the locations. Higher boll retention and opened bolls were observed in 67.5×60 cm and MC (5%) @ 750 ml ha-1at flowering. There was no significant effect of both the treatments factors on node count, monopods and sympods. Highest seed cotton yield was obtained with 67.5 ×30 cm (at Abohar) and 67.5×15 cm (at Faridkot) although these two spacing levels were at par with each other. MC (5%) @ 750 ml ha-1 at flowering resulted in 22.9% and 21.3% increase in seed cotton yield over control at Abohar and Faridkot, respectively. Different plant spacings and mepiquat chloride applications showed a non-significant effect on different fibre quality parameters except halo length at Faridkot. At both the locations, highest B:C ratio was observed in 67.5×30 cm and MC @ 750 ml ha-1 at flowering.

Soil Fertility Dynamics and Xanthomonas Wilt Incidence in Enset (Ensete Ventricosem) Based Farming at Chencha, Southern Ethiopia

Enset (Ensete ventricosum) is a vital food security crop cultivated in South and South-western parts of Ethiopia. However, enset production and the farming families have been threatened by Xanthomonas wilt and its spread in the farming system. Thus, this study was conducted to investigate the soil fertility and plant management practices association on the incidence of Enset Xanthomonas wilt. Data on soils fertility and diseases from enset based farming clustered into inner, outer and outfield farm zone were sampled and surveyed. The result indicated that soil chemical properties significantly (p≤0.05) varied from inner to outfield farm zone. Significantly maximum nutrients store revealed in inner enset farm zones. Disease incidence reduced from inner to the outfield enset farm zone. Disease prevalence and disease incidence scored 28.5% and 11.6%, respectively depending on altitude and genotypes. Soil fertility levels in the enset inner and outfield plots were varied purposely to cultivate enset products as kocho, bulla or amicho (cooking type). The variations in soil fertility and Xanthomonas wilt incidence was associated with management practices applied for desired enset products. Therefore, management practices in enset based farming, soil fertility and location of enset planting zones found to be major indicators for disease incidence addressing to device control interventions.

Radio-Induced Maxillary Angiosarcoma: About a Case

Introduction Angiosarcoma is a rare malignant tumour from fast-evolving and aggressive vascular endothelial cells that accounts for 1% of all sarcomas. The sinus location of angiosarcoma is exceptional. The clinical polymorphism of this tumour may cause a delay in diagnosis. Risk factors are not always understood. However, a history of radiation therapy for locoregional cancer and/or lymphedema is most commonly found. The prognosis remains bleak with an overall five-year survival of 35 to 40%. Objectives The authors discuss the diagnostic, therapeutic and prognostic modalities and difficulties of this tumour. Observation: The authors report a case of maxillary sinus angiosarcoma confirmed by radio-induced secondary CD34 immunohistological staining in a 40-year-old man having a history of hemangioendothelioma treated with conservative endoscopic surgery and adjuvant radiotherapy 15 months earlier. Discussion This lesion is rare in the ENT sphere and may be confused with a benign tumor in its early stages. Only histological examination with immunohistochemistry can confirm the diagnosis (factor VIII, CD34 and CD31 antigens). Treatment consists of a wide resection followed by radiotherapy, hence the need for early diagnosis in the metastatic phase, first-line treatments include doxorubicin or paclitaxel weekly. Conclusion Radio-induced angiosarcoma is a rare malignant tumour, it occurs in patients who have received conservative treatment and radiation therapy for nasosinusitis cancers. Angiosarcomas of the nasal passages remain a diagnostic and therapeutic challenge due to their local and general aggressiveness.

Malaria: A Driving Force to the Emergence and the Global Spread of Antibiotics Resistance

Malaria and bacteraemia are significant public health concerns and economic threats. In Africa, the intensity for simultaneous transmission and co-infection of Plasmodium spp and other bacteria pathogens are extremely high. It is believed that malaria suppress the immune system and enable the translocation of bacteria in the gastrointestinal tract to other cellular compartments in the body. Some of the factors that contributed to the co-emergence of these pathogens are poor access to clean water, sanitation and hygiene (WASH), poor infection control measures, inefficient health care systems. In addition, the similarities in the clinical signs and symptoms of these febrile diseases and the fact that the etiologic diagnostic testing can be complex, costly, and limited are the reasons why clinicians in resource-constrained setting often prescribe antibiotics empirically prior to or without laboratory testing to prevent severe outcomes in any patient hospitalized with malaria. However, this indiscriminate use of antibiotics has been identified as the driving force for antibiotic resistance, which is already at alarming rate in malaria endemic nations. In developed countries where malaria had been previously eradicated, there are increasing reports of imported malaria with concurrent bacteraemia. In this review, we emphasized the role of malaria in the indiscriminate use of antibiotics and the fact that eliminating malaria in Africa is one of the best strategies to address the emergence and the global spread of multi-drug resistance organisms.

Cosmic Surgery: Development of a New Non-Invasive Technique in Modern Surgery.

Cosmic surgery is a non-invasive operation developed by applying modern astronomical discoveries to surgical science. Scientific cosmology is the study of the universe through physics and astronomy. Religious cosmology explains the universe based on religious scriptures. According to religious cosmology, the universe consists of hell, heaven, and earth. Earth is surrounded by hell, and hell is surrounded by heaven. Astronomers explored the universe and found billions of galaxies of stars all around the earth made of hellish fire. The location of the stars and hells in the universe and their characteristics are identical. Comparative study shows that all the stars in the visible galaxies of the universe are religious hells. The discovery of hells in the lower universe confirms the existence of heavens in the inaccessible upper universe that has been predicted in the Holy Scriptures. This astronomical discovery is not only a message to mankind but also a recombinant cosmic signal all over the world, which is generated for the exploration of the cosmos by prophets and astronomers. According to the supernatural theory of disease, supernatural virus infects the prefrontal cortex of the human brain and disrupts the normal spirit of executive functions including reasoning, planning, motivation, and problem solving that leads people to a delusional lifestyle and behavior and alters the dynamic equilibrium between man and environment, and causes all physical, mental, social, and spiritual disorders, national, international, and inter-religious conflicts and wars in the world. Cosmic surgery is a non-invasive operation applying above mentioned cosmic signal to the human brain to sterilize the whole world from the supernatural virus that can change our sick world into a healthier, happier, and prosperous world like a terrestrial paradise.

Study of obstacles of fish culture in Khartoum State (Omdurman, Khartoum and Khartoum north, case study)

The present study aimed to provide baseline information of fish culture status (obstacles) of Omdurman, Khartoum and Khartoum north in the Khartoum state in Sudan due to there is little or lack of information in this field. 30 owners of farm were questioned from Omdurman, Khartoum and Khartoum north, 10 farms for each location; Descriptive analysis was done for analyzing the raw data of the study by using Excel Microsoft Software 2007. Social data showed that male farm owners group was dominant in third sites 100%. Age groups of fishermen ranged between 20 to 60 years; where age group 31- 40 Khartoum north, age group 41- 50 was dominant in O mdurman and age group 51-60 year was dominant in Khartoum. Six educational categories were recorded where University education was dominant in the three locations 50%, 50% and 70% respectively; whereas, postgraduate was the second in the tow location: O mdurman and Khartoum 20% and the secondary education were the third in Khartoum 30%. all farm owners were married 100% in three sites; As regards to the farm owners other activities results showed that most of them were practicing other activities (farmers, business owner, officers and workers). Category of farmer was dominant 90% secondly business owner and officer 70% and 50% respectively. Data showed that the problems (Varanus niloticus, birds, marketing, fees and thefts) was dominant in three locations had the highest percentage 90%, 80%, 60%, 30% and 10% respectively. and according to production cost in three locations arranged: (feeds, workers, fingerlings, water and technical supervision) 50, and 70% and 40%, 50% and 70% and 30%, 40% and 50% and 10, 20 and 40% and 10, 20 and 30% respectively. Category of feeds dominant followed by workers and fingerlings, the best Method to obtain of information, the results showed that Bulletins was dominant in three sites 80%, 30% and 90%; whereas, field visits in three location was the second 60%, 90% and 0%, Radio programs in three location was the third 60%, 10% and 60% and the lowest percentage; Seminars and television 40%, 50% and 40%, and 30%, 0% and 0%. For the best location to obtain information results revealed that the General Director of Fisheries Ministry of Agriculture, Animal Resources and Irrigation - Khartoum State was dominant in the three locations 90%, 80% and 90%, whereas, Universities in three location was the second 40%, 20% and 0%, and the lowest percentage Central Research of fisheries 40%, 0% and 0%, and Internet 10%, 20% and30%. Also result showed most of the farm owners in three sites all them need to training courses in deferent programmers (Fish extinction, Fish feeding, Fish production and Fish marketing) according to this percentages 60%, 30% and 50%, and 30%, 10% and 50%, and 10%, 10% and 30%, and 10%, 0% and 20% respectively.

Genotype x Environment Interaction and Yield Stability Analysis of Some Chickpea (Cicer arietinum L.) Genotypes across Different Environments in Sudan

Genotype by environment (GE) interaction is considered to be among the major factors limiting the efficiency of breeding programs.Five chickpea genotypes were evaluated to study their adaptability and stability in eight environments of Sudan. The experiments were carried out in randomized complete block design (RCBD) with four replications in four locations in over two years (2017/2018 and 2018/2019).Stability was estimated using additive main effects and multiplicative interaction (AMMI) model.A Combined analysis of variance showed significant effects of genotypes, locations, years and their interactions on seed yield. Mean seed yield ranged between 1.88 and 1.45 t ha-1 for the genotypes FLIP 08-59 C and FLIP 09-187 C, respectively. The genotype FLIP 08-59 C gave the highest average seed yield and out-yielded the two checks, Shiekh Mohamed and Burgeig by 9.5% and 10.1%, respectively. The newly introduced (FLIP 08-59 C) showed non- significant differences in earliness as compared to the other two standard checks resistant to fusarium wilt disease and with good seed weight. The statistical analysis showed that genotype (G), environment (E) and their interaction (GEI) had highly significant (P ≤ 0.001) effect on seed yield. It also revealed that environmental differences accounted for 90.2% of the total variation. The results of data analysis displayed that the percentages of seed yield by environment, genotype and genotype environment interaction were 90.2%, 3.6% and 6.3%, respectively. The IPCA 1 and IPCA 2 axes (Principal Components) were highly significant P ≤ 0.001). The model for seed chickpea yield stability (the Additive Main Effect and Multiplicative Interaction (AMMI) model) showed that the G1 (FLIP 08-59 C) was adapted, high yielding and considered as stable genotype for Gezira, River Nile and Northern States of Sudan. In addition, this genotype is early flowering with non-significant differences in earliness as compared to the two checks and with good seed weight. Therefore, the above mentioned genotype FLIP 08-59 C could be recommended as commercially stable and high yielding cultivar and/or incorporated as breeding stocks in any future breeding programs aiming to produce high yielding genotypes of chickpea.

Introduction The idea of HLSSM2 is to get the least easy surgical intervention, as arthroscopic surgery begins with examining the shoulder joint, and the location and size of the lesion are determined, then open over the lesion with an open surgical approach that is very limited for surgical repair; not using arthroscopic instruments but conventional surgical instruments in the second step. Figure 1 Figure 1.   Methods Clinical experience with this technique consists of 44 cases over a period of thirty-one months, this study was from September 2020 to May 2023. All cases were done as outpatients and under general anesthesia. Results The mean duration of the operation was 35 minutes, and the minimum duration was 25 minutes. There was no major nerve or vascular injury in all cases. This technique is simple, safe, and good cosmetically satisfactory for all patients after full recovery about three months after the procedure, and it is cost-effective. One 43-year-old female patient suffered pain and stiffness during the first six months after surgery and was not very satisfied, she did not stop visiting us till now, she was not happy because of shoulder pain and tenderness. Conclusion It can be used by experienced hand surgeons in shoulder arthroscopy and in Mini Lateral Shoulder Approach (MLSA)3, especially in countries where shoulder arthroscopic release and repair with suture anchors are expensive4 or not available. This technique is not very simple but is available, safe, cosmetically satisfactory, and cost-effective.

Evaluation of selection indices for heat tolerance and their correlation with yield in some chickpea (Cicer Arietinum L.) genotypes of sudan

The major a biotic stresses affecting chickpea production are high and low temperature, drought and salinity. Heat stress is a major a biotic stress factor, constraining chickpea production worldwide. This study was conducted to identifying chickpea genotypes combining heat tolerance and high yield potential and to study correlation among the heat tolerance indices. The genotypes were tested under different field-growing conditions, normal sown (non – heat stress) and late sown (heat stress) at two locations, Merowe and Gezira during winter season 2018/2019. The trials were laid out in alpha lattice design with three replications. Eight heat tolerance indices, which were most frequently used in plant breeding including, geometric mean productivity (GMP), yield index (YI), mean productivity (MP), stress susceptibility index (SSI), stress tolerance index (STI), tolerance index (TOL), sensitivity heat index (SHI) and relative heat index (RHI) were calculated based on seed yield under heat stress (Ys) and non – heat stress (Yp) conditions. Under both environments, the results from combined analysis of variance showed that there were highly significant differences among the genotypes for all traits studies. There were highly significant differences observed among the tested chickpea genotypes response to heat stress. Seed yield in stress (Ys) condition was positive and highly significant correlated with geometric mean productivity, yield index, stress tolerance index and relative heat index. The Ys was negative and highly significant correlated with stress susceptibility index and sensitivity heat index. Seed yield in non-stress (Yp) condition was positive and significant correlated with geometric mean productivity, yield index, stress tolerance index and tolerance index. The results of this research showed that the six indices, GMP, YI, SSI, STI, SHI and RHI can be used as optimal indicators for screening heat tolerant chickpea genotypes. Nine genotypes including four released improved varieties (Shiekh Mohamed, Wad Hamid, Salwa and Hwata) and five genotypes (no. 11, 4, 26, 30 and 34) are most stable chickpea genotypes for heat tolerance and can be further used in breeding program. These genotypes can be used in the chickpea national breeding program to develop varieties with improved heat tolerance in Sudan. Genotype no. 11 (FLIP 08-59 C) was release by Sudan national variety release committee for commercial production under the name Elbarkal in September 2022 to will be grown in Gezira, River Nile and Northern States of Sudan.

Abdominal Aortic Aneurysms: Epidemiological, Clinical and Echographic Aspects in The Health Structures of Lomé and At The Kara University Hospital (TOGO)

Objective To describe the epidemiological, clinical and arterial Doppler ultrasound aspects of abdominal aortic aneurysm (AAA). Material and Methods This is a prospective study with descriptive and analytical aims carried out from January 2019 to March 2022 (3 years) at the cardiovascular exploration center (CEC) "Saint-Esprit" of the association of the people's doctors for the research on cardiovascular diseases (AMP-MCV), at the clinic "Le Printemps" and at the University Hospital of Kara. All patients who underwent arterial Doppler ultrasound of the lower limbs for any reason in these centers during the study period were included. Ultrasound scans were performed or confirmed by a physician with a diploma specializing in vascular Doppler ultrasound. Results We retained 32 Doppler ultrasound scans out of 739 performed for AAA with a frequency of 4.38%. The mean age was 73.5± 9.07 years. The sex ratio M/F was 1.4. The cardiovascular medical history (84.38%) found was: hypertension (33.33%), diabetes (18.52%), dyslipidemia (33.33%) and a combination of diabetes and hypertension (14.81%). The circumstances of discovery were: downstream MI ischemia (72%), incidental finding on abdominal ultrasound (15.63%) and De bakey sign (6.25%). The location of the aneurysms was subrenal (84.4%), suprarenal (9.4%) and both (6.2%). AAAs were fusiform (71.9%) and saccular (28.1%). The size of the aneurysms was greater than 5 cm in 78.17% of patients. High blood pressure increases the probability of the occurrence of subrenal AAA while dyslipidemia increases the occurrence of fusiform AAA. Diabetes increases the probability of occurrence of AAA larger than 7cm. AAAs were thrombosed in 75% of patients. There were extensions of the aneurysms to the iliac arteries in 70.59%. Downstream embolic complications represented 46.88%, in the form of occlusions and tight stenoses. Spontaneous mortality was 81.24%. Conclusion Arterial Doppler ultrasonography is a very important tool in the screening, positive and lesion diagnosis of AAA. The risk factors for AAA are: age over 60 years, male gender, hypertension, diabetes and dyslipidemia. Early detection remains essential, because the spontaneous prognosis is poor.

Histo-Morphological Effect of The Small, Large Intestines and Stomach of Animal Models Treated With Aqueous Extract of Abelmoschus Esculentus

This study investigates the effect of Aqueous extract of abelmoschus esculentus on the microanatomy of the small, large intestine and stomach and the body weight of Wister rats. Twenty-one adults male wistar rats weighing between 100-120 grams were assigned into three groups consisting of seven rats each; Group A (control), Group B (low dose), and Group C (high dose). The rats in the control group were fed with fed with feed and water only while the rats in groups B and C were treated with 0.1mg/kg body weight and 3.0mg/kg body weight of abelmoschus esculentus respectively for 14 days. At the end of administration, the final weights of all rats were recorded before sacrifice using cervical dislocation and the small, large intestine and the stomach were harvested, processed and stained using H&E stain. The results were revealed as significant (p<0.05) increased in the mean body weight compared with the weight in the control groups and experimental groups. The treated animal groups revealed increased cellularity, focal metaplasia of the mucosal cells with villous disruption in the small intestine and dysplasia of the mucosal with loss of epithelial shape in large intestine. The stomach histology showed gastric pits with goblet cells smooth muscles layer and surface epithelium in the control group. Sections from the low dose treated group showed deep epithelical gastric pit areas with marked depletion of pits and goblet cells while the high dose treated group revealed dysplasia of gastric pits, goblet cells and smooth muscles appear mildly eroded.

Modeling of the Deposition of Low-Density Lipoprotein Particles inside an Ideal Geometry of the Heart Coronary Arteries

In this study, flow hemodynamic parameters and settling of LDL particles in healthy ideal and ideal with stenosis coronary artery have been studied using the Open FOAM software. Not only the effect of movement of the vessel on particles depositions have been studied, but also both non-Newtonian and Newtonian behaviors of blood have been investigated. The particle's motion and deposition have been simulated. It has resulted that only in small vessels, non-Newtonian behavior of blood can be seen and in bigger vessels, there is no remarkable difference between two Newtonian and non-Newtonian models. Vessel movement has no important effects on entered shear stress on its wall, but the alteration in flow lines causes a 70% increment in particles depositions on a vessel with stenosis. This shear stress has no considerable changes with inlet velocity alteration, but it has intensive dependence on geometry in the vessel with stenosis (13 times more oscillation in stenosis location than a healthy vessel).

Integrated Management of Sclerotinia Sclerotiorum, An Emerging Fungal Pathogen Causing White Mold Disease

Sclerotiniasclerotiorum, the causal agent for white mold (Sclerotinia stem rot), is a devastating fungal pathogen. Currently, Sclerotinia is most commonly managed using the chemical fungicide which can lead to Sclerotinia resistance development, impacting biodiversity and interfering with key ecosystem services. In this regards, field experiments were conducted during 2017-18 planting seasons to evaluate the efficacy of different components viz. sawdust burning, stable bleaching powder, fungal and bacterial bio-control agents, chemical fungicide Rovral 50 WP and integration of different components for the management white mold disease of bush bean, mustard and garden pea in three different locations viz. in the field of Plant Pathology Division, Bangladesh Agricultural Research Institute, Joydebpur, Gazipur, Regional Agricultural Research Station (RARS), Burirhat, Rangpur and RARS, Ishurdi, Pabna, respectively. The results showed that different treatments displayed varying levels of effectiveness against the disease. All the treatments gave satisfactory reduction of white mold disease development and increased plant growth as well as yield of bush bean, mustard and garden pea. Among the treatments, integration of saw dust burning + soil amendments with Trichoderma based bio-fungicide + bacillus based bio-control agents + application fungicide Rovral 50 WP is the best treatment which reduced 97.49%, 77.72%, 72.26% white mold disease incidence and 84.61%, 81.14%, 71.01% white mold disease severity of mustard, bush bean and garden pea, respectively and increasing plant growth parameter as well as 52.16%, 27.74%, 36.97% yield of mustard, bush bean and garden pea, respectively. Application of only fungicide Rovral 50 WP also better treatment in reduction of white mold disease incidence and disease severity and increasing plant growth parameter as well as increasing yield of mustard, bush bean and garden pea. Soil amendment with fungal or bacterial bio-control agents also gave satisfactory results in reduction of white mold disease incidence and disease severity and increasing plant growth parameter as well as increasing yield of mustard, bush bean and garden pea. It could be concluded from the obtained results that integration between bio-control agents as a soil treatment and foliar application chemical fungicide might be useful as a good tool for controlling white mold disease caused by S.sclerotiorum and obtained higher yield of bush bean, mustard and garden pea under field condition.

Fine and Ultrafine Particle Pollution Before and After a Smoking ban in the Catering Industry in Vienna

In the catering industrytobacco smoke was the primary source of fine and ultrafine particles, which are well known for their health-damaging effects. As shown in studies, attempts to reduce passive smoking in the catering industry of Vienna, like separated smoking rooms, failed to reduce fine and ultrafine particle concentrations effectively. On November 1st 2019, an enlarged non-smoker’s protection law was introduced, including a total smoking-ban in the catering industry. 40 hospitality venues with areas for smokers and non-smokers before the ban had been selected as typical Viennese cafes, pubs, bars and discotheques to be sampled unannounced. Concentrations of fine particle mass (PM10, PM2.5, PM1) and ultrafine particle number (PNC) and lung deposited surface area (LDSA) could be measured before and after the introduction of the smoking-ban in 39 venues at nearly identical locations and under comparable circumstances. Results showed a statistically significant decline in both fine and ultrafine particle concentrations in the former smoking areas for all parameters as well as in the former non-smoking areas for PM2.5, PM1 and LDSA. After the ban concentrations in former smoking areas and non-smoking areas showed no significant differences any more. From these results the smoking-ban successfully removed particles from breathing air of guests and staff, however, some outliers in the study after the ban point to the necessity of repeated controls in Vienna. Also, outside Vienna the compliance with the law should be controlled in the Austrian hospitality industry.

Uptake of Breast Screening Among Female Staff at A Tertiary Health Institution in South-West Nigeria

Background Breast cancer is one of the most common types of cancer affecting women globally. It has shown increasing morbidity and mortality rates over the past years. Several screening methods ranging from simple breast self-examination to a highly sensitive test procedures for early detection and treatment are available, but uptake is a challenge. Lack of a national screening programme in the country makes available regional screening programme unsuccessful. This study aims to assess the factors affecting the uptake of breast cancer screening programme among female staff in a tertiary hospital, Southwest Nigeria. Method This descriptive cross-sectional study involves 375 participants selected by a stratified random sampling technique, with proportional allocation to population size and the use of a semi-structured questionnaire. Respondents were questioned on their basic knowledge of breast cancer symptoms, risk factors and treatment, over a score of 50 and above, were set as the cut-off mark to determine good knowledge of breast cancer. Patients attitudes towards breast cancer screening as well as factors affecting uptake were also measured. Descriptive statistical analysis was done using SPSS-20 while the predictors of the uptake of screening were determined using logistic regression at p ≤ 0.05. Results There were 360 respondents, comprising of 13.3% clinical and 86.7% non-clinical female staff within the age bracket of 20 to 58 years (38.2±0.42 years). Out of the respondents, 97.8% have heard about breast cancer but only 52.3% had been screened while 65.5% had “Good knowledge” of breast cancer. Positive attitude to breast cancer screening was displayed in 52.5% while over 90% claimed to be practicing breast self-examination. Only 36.1% of respondents above 40 years old have had mammography done. Most frequent barriers to screening uptake include cost, poor accessibility to screening facilities, shyness (unfamiliar screener), unavailability of female doctors, careless attitude, fear of cancer and other more pressing family problems. Using logistic Regression at p ≤ 0.05, females with a negative attitude, young unmarried (less than 30years) and lack of easy accessibility to screening facilities were significant factors affecting uptake of breast cancer screening services. Conclusion Due to knowledge-uptake gap of breast cancer screening revealed in this study. It is therefore obvious from these findings that a concerted effort is needed to actively remove these barriers by repeated education, training and re-training strategies among health workers and to improve their uptake and level of advocacy and campaign for breast cancer screening among their patients and clients.

Socio Economic Profile and Nutritional Status of the Rickshaw Pullers of Dhaka City Along with their Energy Intake Gap

Background This study focused on nutritional status of the rickshaw pullers, their dietary energy and nutrients consumption along with their energy intake gap as compared to expenditure. Methods A cross sectional study was carried out at 15 different important locations of Dhaka city during December-January 2018. In total, 670 rickshaw pullers from these locations were randomly selected. Printed predesigned and validated semi structured questionnaire was used to record necessary information. Nutritional status was determined according to Asia specific BMI cut- off values. Energy expenditure was calculated using their Basal Metabolic Rate (BMR) and Physical Activity Level (PAL) value. 24-hour dietary recall method was administered for dietary analysis. Results About 65.4% of them were found within normal BMI range. About 11.5% were underweight and about 23.1% were found having more than normal BMI. Percentage of both underweight (39.0%) and overweight (35.8%) were found higher among those who were between 25 and 35 years. Average daily dietary energy consumption was 2391 kcal and energy expenditure were found 4608 kcal. Their energy consumption was found 2217 kcal less compared to their energy expenditure. Conclusion The current study revealed the nutritional status, dietary intake of the rickshaw pullers of Dhaka city and found energy gap in their dietary intake compared to their expenditure. For their better health and nutrition outcome integrated approach should be taken by the responsible authorities.

Favourable Outcome in Pediatric Neurosurgery for Extra Dural Hematoma in Sub Sahara Africa: Report of Parakou University Hospital in the Benin Republic

Introduction The extra dural hematoma is a blood collection between the cranial vault and the dura mater. It is a neurosurgical emergency whose statistical data are poorly known in the Republic of Benin. Objective The aims of this study are to assess epidemiology, therapeutic and outcome of traumatic extra dural hematomas for children admitted to the surgical department of CHUD-B/A from 2012 to 2017. Methods This was a cross-sectional, descriptive and retrospective study over a period from January 1st, 2012 to December 31st, 2017. Results One hundred and seventy-eight cases of children aged between 0-15 years were examined for brain traumatic injury. Twenty-seven 27 (15.2%) cases of extra dural hematomas were retained. The most represented age group was between 11-15 years with a frequency of 42.1%. The male sex was more represented (78.9%). Road traffic accidents were the main aetiology (57.9%). 52.6% had a mild head injury, 26.3% had a moderate head injury and 21.1% had a severe head injury. On brain scan the frequently observed location was temporo-parietal (31.6%). Acute anemia was observed in 16 children. Seventeen children were operated on. The operation involved evacuation of extra dural hematoma by cranial bone flap with suspension of the dura mater. There were no deaths. Conclusion The availability of brain CT makes the diagnosis of extra dural hematoma easier. Its management is neurosurgical with a favourable post-operative evolution in all cases in this series.

Perceived Effect of Knowledge Level and Socio-Demographics on COVID-19 Risk Exposure: the Africa Experience

COVID-19 remains a global public health emergency till date. It is eminent that the transmission of the disease is subjective to people’s readiness to implement public health preventative strategies and these are often related to knowledge. Proper public knowledge about COVID-19 plus its predisposing factors is critical to effectively manage the increasing public health risks. However, socio-demographics have been implicated to COVID-19 infection risk and management outcome. Thus, this present study examined the influence of knowledge on COVID-19 risk outcome, the contribution of socio-demographics on the risk of COVID-19 and predicted synergistic effects of knowledge and socio-demographics on the risk of COVID-19. All measured was strictly perception amongst African sampled with an online Google form as the primary data source. The Correlation designed used Zr Statistics of Fisher Transformation to determine the differences between the two correlation coefficients of the prediction variables after an initial test using Pearson Product Moment Correlation between COVID-19 risk and Knowledge plus socio-demographic. The hypothesis was tested using Statistical Package for Social Science version 21 and iStat at varying significant levels of 0.05 and 0.01 respectively. Generally, a significant relationship exists between COVID-19 risk and knowledge level but not with composite socio-demographics. However, specific significant relationship (p<0.05) was noticed between COVID-19 risk and age (r=0.220) as well as marital status (r=-0.158). Educational level, location, and sex showed no correlation (p>0.05) with COVID-19 exposure. Also, the proposition of no significant difference between correlation coefficients of socio-demographics and knowledge was proven otherwise (p<0.05). The regression model (R2=0.420 and adjusted R2=0.404, df=2, 336, F-value=27.012. p=0.00) significantly predicted the synergic contributions of knowledge level and socio-demographics to COVID-19 exposure. 40.04% of the COVID-19 risk exposure can be explained by socio-demographics and knowledge about COVID-19. Synergic contribution of knowledge and socio-demographics proved risk prediction to COVID-19. Traditional factor like age should be decidedly considered and attention should be drawn towards good knowledge about COVID-19 especially its signs and symptoms plus transmission.

Reversible Posterior Encephalopathy Syndrome and Related Factors: Clinical Cases Study

Background Reversible posterior encephalopathy syndrome (RPE) is a clinical and radiological entity characterized by the acute or subacute fitting of symptoms covering headache, vomiting, visual disturbances, seizures and impairment of consciousness. The pathophysiology of RPE syndrome is poorly described. RPE syndrome is characterized by a reversible cerebral edema of often posterior topography in magnetic resonance imagery (MRI). Cases Presentation We consider RPE syndrome four cases under various conditions that are known as airplane flight, hypertension, non-steroidal anti-inflammatory medication, pregnancy and oldness with several pathologies. The RPE was described with several symptoms like headaches, vomiting, focal motor deficit, paresthesia, seizures, disorders of consciousness and photophobia. The imagery findings were varying from cortical hypersignals in Flair sequences to edema of both cortex and sub cortex. The outcome was good with a complete regression of symptoms and imagery lesions. Conclusion The pathophysiological mechanism of RPE syndrome remains unknown. High blood pressure, renal failure and drugs (anti-depressants, NSAIDs, immunosuppressants) are the most etiological factors. The diagnosis is based on clinical arguments and brain MRI. The main location is posterior. The clinical outcome was good with all the patients in our study, no recurrence was noted.

Investigation of Emerging Risk Factors and Isolation of Potential Pathogenic Bacteria from Domestic Dog Stool in Port Harcourt Metropolis, Niger Delta

This study investigated the potential pathogenic bacteria that are associated with domestic dog stool in Port Harcourt Metropolis. Convenience sampling techniques were used for sample collection outcome. A total of fifty dog stool samples were collected aseptically into a sterile stool container from different locations (Agip Estate, Rumuokoro Community, Government Residential Area, Sand-fill Borokiri, Port Harcourt Township, and Rivers State University lecturers’ quarters) all within Port Harcourt City. Bacteriological analysis was determined using standard microbiological analytical and identification techniques. Statistical analysis was performed using Statistical Package for Social Science version 21 for frequency, percentage, prevalence rate and correlation at 0.01 and 0.05 level of significance. Potential pathogens with associated percentages that were isolated are E coli (20.0%), Klebsiella species (16.0%), Pseudomonas species (4.0%), Proteus species (28.0%), Bacillus species (4.0%), Staphylococcus aureus (14.0%), and unidentified [other Staphylococcus species] (4.0%). However, the research further revealed that Proteus species (28.0%), was the most prevalent pathogen, while Bacillus species (4.0%), Pseudomonas species (4.0%), and Streptococcus species (4.0%), were the least prevalent among pet dogs studied. The correlation analysis showed no significant relationship between isolates with socio-demographic data of pet owner and Pet biodata respectively. From the analysis, correlation coefficient values of .269 and .124 were obtained for Age of Pet Owner and Gender of Owner respectively. It also show a direct correlation but by implication, a very weak, non-significant (p<0.05) relationship existed between the organisms isolated and the variables (Age of Pet Owner and Gender of Pet Owner). However, no relationship exists between isolates and visit to vet Doctor (r-0.038; p>0.05). Furthermore, negative non-significant correlation (p>0.05) was observed for marital status of Pet Owner (r=-.158) and Age of Pet (r=-.023) and isolate. Also, correlation association between isolates and some risk factors using the correlation analysis matrix revealed an indication of an indirect but moderate association between isolates and Caressing (r=-.347, p=0.01). However, other risk factors like vaccination (r=.042), Feeding Pattern(r=.125), Pet Bathing (r=.220) and Eating with pet in same plate (r=.146), did not prove statistical significance at p>0.05. The results from the study strongly suggest that pet dogs carry potential pathogenic organisms in their faecal matter that can serve as a source of infection to the pet owners. It is, therefore, very critical that these pets should be treated and possibly vaccinated frequently, even as their faecal matters should be well disposed to prevent possible zoonotic infectious epidemic outbreak in our global communities.

Effects of Aluminium Sulphates on Weight and Structure of Adrenal Gland of Adult Wistar Rats

This study evaluated the effects of aluminum sulphate exposure on the histology of adrenal gland of Wistar rats. Thirty adult Wistar rats were used for this study. The Wister rats were divided into three groups; group A was the control animals and tagged C, group B animals received 10g of alum dissolved in 1000cm3 of distilled water and were tagged T1, group C animals received 50g of alum dissolved in 1000cm3of distilled water and were tagged T2, via drinking water for duration of four weeks. Twenty-four hours after the last administration, the rats were sacrificed by cervical dislocation. The adrenal gland was excised and preserved in 10 % formosaline after which it was routinely processed for hematoxylin and eosin staining (H&E). Histological observations showed normal cell distribution in the control group but treated group revealed evidences of cellular obliteration& hemorrhagic necrosis. The results obtained from this study suggest that aluminum sulphate has a damaging effect on the structure of the adrenal gland.

Bacteriological Quality of Groundwater in Imiringi Town, Bayelsa State, Nigeria

This study evaluated the bacteriological quality of groundwater from Imiringi in Ogbia Local government area of Bayelsa State, Nigeria. Groundwater samples were obtained in triplicates from four locations in the study area and analyzed for the bacteria quality using standard protocols. Results showed that the microbial density ranged from 2.97 – 6.03 x 104 cfu/ml (for total heterotrophic bacteria counts), 4.17 – 5.43 x 102 cfu/ml (for Enterobacteriaceae bacteria counts) and 0.00 – 1.48 x 102 cfu/ml (for Salmonella-Shigella counts). Statistically, there was no significant difference (p>0.05) across the various locations for each of the parameters. The density observed were higher than the allowable limit of 1.0 x 102 cfu/ml for drinking water as recommended by World Health Organization/Food and Agricultural Organization, and maximum permissible level of 10cfu/ml for total coliform as specified by Standard Organization of Nigeria. The prevalence of bacteria isolated in the water samples were Aeromonas species (8.00%), Citrobacter species (7.50%), Escherichiacoli (20.75%), Klebsiella species (5.50%), Pseudomonas species (12.00%), Salmonella species (2.25%), Shigella species (3.25%) and Staphylococcus species (40.75%). From the bacteria density and diversity, the water is not potable. Some of the bacterial found are of public health importance. Hence, there is a need to adequately treat the water using chlorination and boiling techniques before drinking.

Perineurioma is an exceptional, benign neoplasm entirely composed of neoplastic perineurial cells and arises from the peripheral nerve. Pertaining to location, the neoplasm predominantly demonstrates configurations such as intra-neural or extra-neural, confined to the soft tissue. Perineurioma was initially scripted by Lazarus and Trombetta in 1978 and is additionally nomenclated as localized hypertrophic neuropathy1. Extra-neural soft tissue perineurioma and mucosal (intestinal) perineurioma are frequent, in contrast to intra-neural perineurioma or localized hypertrophic neuropathy subtype12.

Distribution Spread and Environmental Risk Status of Pb, Cd And Cr in Soils of an Open-Air Waste Dumpsite along Tombia/Amassoma Road in Yenagoa Metropolis

In spite of the popularity of open-air waste dumping in Nigeria, it remains a relatively less effective waste management option across the globe because of its associated environmental impacts which includes the release of green house gases (GHGs), persistent organic pollutants (POPs), and metal micro-pollutants amongst others. This study aims to assess the potential environmental risks associated to metals released and vertically delineated across the soil profile within surroundings of dumpsite. Heavy metals in soil samples were acid-digested using the aqua-regia mixture of hydrochloric and nitric acid, followed by instrumentation analysis using the GBC 908 PBMT model atomic absorption spectrophotometer. Contaminated sites showed metal concentrations ranging from 1.493 to 109.460 mg/kg, 0.133 to 4.237 mg/kg, and 5.200 to 25.367 mg/kg for lead, cadmium and chromium respectively, with location 1 land area showing the most contamination. Only soil chromium was observed within regulatory stipulations in all cases. There was significant variation (p < 0.05) between the different sample locations, thereby indicating variations in composition of dumped wastes. Lead and cadmium showed the strongest positive correlation (r = 0.855, p < 0.01) and the application of some heavy metal pollution indicators revealed relatively higher metal loads and degree of contamination, as well as depicting potential ecological risk for soils of location 1. The significant heavy metal contamination of soils in the Tombia-Amassoma waste dumpsite requires that the local environmental sanitation and regulatory authorities take necessary remedial action to forestall the escalation of public health concerns that may emanate from this open-air dump.

Case Series on Chiasmal Lesions with Ocular Manifestations seen at the Eye Center of a Tertiary Government Hospital in Philippines

Objective To identify common ocular and non-ocular signs and symptoms of patients with chiasmal lesions presenting to ophthalmologists.  Methods This is a three year case series of patients who presented to Rizal Medical Center Ophthalmology OPD clinic, diagnosed as cases of chiasmal lesion with a complete neuro-ophthalmogical exam, ancillary test and neuroimaging. Data collected include patient profile, ocular and non ocular symptoms, duration of blurring of vision (BOV), best corrected visual acuity (BCVA), Color Vision, Optic nerve (ON) description, Visual field defects (VFD) and size & location of chiasmal lesion.  Results A total of twelve patients were included with 1:1 M:F ratio and a median age of 39 years. Blurring of Vision was the presenting symptom in 10 cases, bulging of the eyes and headaches in the remaining cases. Associated symptoms include: non specific headache in all; diplopia in 3 (1 with and 2 without motility defects); and systemic symptoms related to hormonal imbalance in 2 cases. Best corrected vision ranged from 6/6 to no light perception (NLP). Color vision was affected in all cases. ON palor was seen in 6 (50%) and disc edema in 1 case. VFD include Junctional scotoma (58.33%), bitemporal hemianopia (33.3%) and generalised scotoma (8.3%). Neuroimaging revealed pituitary lesions in 10 cases, 1 craniopharyngioma and ON glioma with chiasmal extension. Conclusion Ocular and non ocular symptoms seen were comparable to other studies except that Junctional scotoma was the most common VFD identified in the included population of our case series. This highlights the importance of VF testing among patients complaining of unilateral visual loss to rule out a chiasmal lesion.

Evaluations of phylogenetic proximity in a group of 67 dogs with osteosarcoma: a pilot study

Aim of the Research The aim of this study is to analyze a group of dogs of different breeds affected by osteosarcoma (OSA), to document any prevalence of this primary bone neoplasia in breeds that are phylogenetically close and to help with further research human medicine, as a model of study for prevalence and epidemiology of human OSA in multiple populations. Study Design Pilot study from two canine surgery databases between 2002 and 2013. Materials and Methods Breeds were classified in groups based on their phylogenetical proximity. Differences in prevalence of OSA between breeds and groups were evaluated with a permutation test. For each breed and each group, a ranking was made by calculating 95 % confidence intervals and counting the no-overlapping between breeds and groups. The relation between the dogs’ heights and the prevalence of OSA was analyzed using a logistic regression between the disease status and dog size. Results A total of 67 dogs with OSA, in 26 different breeds were included. Ten breeds were overrepresented and, the majority of these, were classified in 2 predisposed groups phylogenetically close to each other. The prevalence of OSA was associated with the dogs’ height within predisposed breeds, but, in general, taller breeds were not the most affected. Conclusions and Clinical Relevance In this study, despite the small number of dogs, we observed that the most commonly affected breeds with OSA are phylogenetically closely related. This highlights the importance of genetics in the aetiology of canine OSA . In this preliminary study, indications are given on breeds, samples and genome locations to be further investigated. This could allow identification of pathogenic alleles in dogs, and potentially in humans. Furthermore this pilot research can represent a model of epidemiologic study of human OSA.

Assessment of Heavy Metals Contamination of Water and Cupscale Grass (Sacciolepisafricana) along the Epie Creek in Bayelsa State, Nigeria

This study was conducted to investigate the concentration of three heavy metals; iron, chromium and lead in surface water and plant (Sacciolepisafricana) tissues along the Epie creek. This was aimed at determining the heavy metal loading of surface waters and the extent of bioaccumulation in the most prevalent grass species found growing within the river course. Metals were determined using atomic absorption spectrophotometer. Result of water analysis recorded the least and most significant iron concentrations of 3.42 ± 0.06 mg/L and 12.06 ± 0.06 mg/L for Agudama and Edepie axis respectively. Contrastingly, water samples showed chromium and lead levels which were below their instrument measurable limits. Consequently, lead (Pb) and chromium (Cr) levels were within WHO permissible limits, while iron (Fe) exceeded regulatory standard for water samples collected from all field locations. Similarly, iron recorded elevated iron concentrations in plant (cupscale grass) with the least amount being revealed for Igbogene stem (196.63 ± 5.21 mg/kg), and the most concentration been reflected for Edepie root sample (9,423.17 ± 48.55 mg/kg). Even though iron levels prevalently exceeded recommended threshold in plants, the stem were mostly within limit for iron. The only grass sample depicting significant lead amount (0.34 mg/kg) was the stem section collected at Akenfa location. Evidently, metals were least stored in plant stems and most bioaccumulated in plant roots. Furthermore, Cr and Fe are the only significantly correlating metals in plants (r = 0.700, p < 0.01). Overall, the increasing level of Fe in the water environment is further indication of the impact of unregulated dumping of scraps and domestic waste along the creeks embankment.

Uptake of Cervical Cancer Screening Among Female Staff at the University College Hospital, Ibadan

Background This study aim to assess the factors affecting uptake of cervical cancer screening programmes among female staff of the University College Hospital (UCH), Ibadan in 2014. Method A descriptive cross-sectional study involving 375 participants selected using a stratified random sampling technique with proportional allocation to population size and interviewed using a pre-tested self-administered semi-structured questionnaire. Information on their basic knowledge on cervical cancer were scored to determine good knowledge. Patients’ attitudes toward cervical cancer screening as well as factors affecting uptake were also assessed. Data analysis was done in 2014 using descriptive statistics while screening uptake predictors were determined using logistic regression at p ≤0.05. Results Completed questionnaires were returned by 360 respondents; 13.3% were clinical while 86.7% were non-clinical staff of age range 20 to 58 years (38.2±0.42 years). Majority of the respondents, (95.5%), had heard about cervical cancer with 61.9% having “good knowledge”, but only few (34.2%) had been screened. Averagely, 47.5% displayed positive attitude to cervical cancer screening with Pap smear being the most reported screening procedure. The common barriers to screening uptaking include indecision, inadequate information and feeling of good health. Others were staff hostility, lack of privacy, and cost. Using logistic Regression at p ≤0.05, females with negative attitude and those with children were less likely to uptake cervical cancer screening services. Conclusion The knowledge-uptake gaps of cervical cancer screenings were high as revealed in this study. Therefore, there is a need to further educate eligible women on uptake of cervical cancer screening.

Maternal Indigenous and Artisanal Coastal Nutrition, the SDG Imperative: A Suggested Renaissance of Ethics for Research and Tertiary Education in the Anthropocene Era

Specifically, there are situations all around the globe where Indigenous and Artisanal coastal people suffer from maternal malnutrition inadvertently limiting future potentials in many locations that will be most challenged by climate change. Results from research with Artisanal Fisherfolk in the Philippines and analysis of harvest by the Canadian Inuit people are discussed in terms of the ethics of setting national as well as global education and research priorities.

Phytoplankton Community of Taylor Creek in the Niger Delta Using Diversity Indices

This study evaluated the phytoplankton community of Taylor creek from Polaku to Agbia between November 2013 and July 2014. Sampling was carried out in 12 locations along the stretch of the creek following standard protocol. The phytoplankton enumeration was done and identified accordingly. In all the 12 locations, 100 species of phytoplankton belonging to 14 taxonomic groups including Bacillariophyta (31 species), Chlorophyta (25 species), Pyrrophyta (4 species), Cyanophyta (17 species), Spermatophyta (9 species), Euglenophyta (3 species), Phaeophyta (1 species), Heterokontophyta (2 species), Myxophyta (1 species), Xanthophyta (1 species), Chrysophyta (2 species), Lycopodiophyta (1 species), Cryptophyta (1 species), Dinophyta (2 species). Bacillariophyta, Chlorophyta, Cyanophyta and Spermatophyta with occurrence rate of 36%, 34%, 10% and 10%, respectively were the predominant phytoplankton in the study area occurring in all the locations across the period of the study. The various species have some distinct environmental condition that enable them thrive in their niche, and this determines the structure of the phytoplankton in the study area. The diversity indices provided information about the distribution and health condition of the creek. The study showed significant relationships between Shannon-Wiener, Menhinick, Margalef, equitability and Fisher-alpha indices. The ecosystem showed moderate to heavy pollution based on Shannon Wiener index. There is the need for improved method of waste disposal and other anthropogenic activities being carried in and/ or within the creek.

Exploring the Feasibility of Supporting UK Partners Living Alongside Veterans with PTSD: A Pilot Study of the Together Programme (TTP)

Background Romantic partners living alongside veterans with Post Traumatic Stress Disorder (PTSD) appear at increased risk of secondary traumatic stress (sPTSD) and common mental health difficulties (CMD) compared to the general population.  The severity of symptoms implies the need for structured, bespoke and evidence-based interventions. Objective The aim of this study was to explore the feasibility of offering a community support programme (The Together Programme, TTP) for military partners. TTP was developed based upon a number of US programmes and consisted of 10 hours of group-based support delivered over a five-week course. 56 participants engaged in TTP over a year at nine locations across the UK and were followed up three months later.   Methods Measures of CMD, sPTSD, alcohol use and relationship satisfaction were used to assess benefits. Data were also collected on attendance and participant feedback. Results Significant reductions were observed for symptoms of sPTSD and CMD at follow up. 51/56 (90.1%) participants completed TTP.  The majority of participants reported positive experiences. However, several individuals stated wanting more sessions and that barriers such as work, and family commitments made it difficult to attend. Conclusions Whilst limitations exist, the data presented suggests cautious optimism for the efficacy of offering a structured programme of support to address the needs of military partners living alongside PTSD.

Environmental and Health Risk Assessment of Particulate Matter Associated with Dusty Football Field

Over the decades there have been problems associated with Particulate Matter (PM) arising from anthropogenic activities. Evaluation and health risk assessment of PM associated with football field was carried out in 5 sampling locations, including the control station. Digital portable AEROCET 513 (Metone instrument) PM meter was used for the evaluation Results showed levels of PM for; PM1.0(13.73 – 20.18 µg/m3), PM2.5 (19.11 – 28.83 µg/m3), PM4.0 (24.73 – 44.63 µg/m3), PM7.0 (41.07 – 67.04 µg/m3), and PM10 ranging from 65.48 – 90.82 µg/m3. In addition, value of the control station was lowest amongst all recorded values of PM (p<0.05). Based on Air Quality Index evaluation, the football fields were predominantly rated as unhealthy, with other cases rates as; hazardous and unsafe for sensitive group. This investigation therefore concludes that the emission of PM from the football field are reflection of vehicular emissions and agitated dust particles. Hence mitigable measure must be applied without much ado, in order to ameliorate the daily chronic exposure to PM.

Assessment of Water Quality of Blue Nile River in Sudan

The objectives of this study were to quantify the fresh water quality of Blue Nile River before processing, identify the pollutants, and to determine the most polluted areas, and their impacts on living organisms as well as the surrounding environment. Thus, random water samples were collected and analyzed at the laboratory of the Ministry of Irrigation and Water Resources, Ground water and Wadis Directorates - Khartoum. The outcomes were compared with the World Health Organization standardization. The results revealed variations in the concentration of the studied elements taken from the different locations. But, the results indicated that the water quality is good, and it is within the permissible water use. However, further study is recommended to include seasonal variation as well as the biological analysis.

Society may understate the risk to individuals living near turbines due to an overstatement of the perceived benefits of turbines, and an understatement of the risk of injury from falling turbine parts, or shed ice. Flaws in risk calculation may be attributed to a less than fully developed safety culture. Indications of this are the lack of a comprehensive industry failure database, and safety limits enabling the industry growth, but not protective of the public. A comprehensive study of wind turbine failures and risks in the Canadian province of Ontario gives data to enable validation of existing failure models. Failure probabilities are calculated, to show risk on personal property, or in public spaces. Repeated failures, and inadequate safety separation show public safety is not currently assured. A method of calculating setbacks from wind turbines to mitigate public risk is shown. Wind turbines with inadequate setbacks can adversely impact public health both directly from physical risk and indirectly by irritation from loss of safe use of property. Physical public safety setbacks are separate from larger setbacks required to prevent irritation from noise and other stressors, particularly when applied to areas of learning, rest and recuperation. The insights provided by this paper can assist the industry to enhance its image and improve its operation, as well as helping regulators set safety guidelines assuring protection of the public.

Psychosocial Determinants of Marijuana Utilization among Selected Junior High School Students in the Central Region of Ghana

Marijuana utilization among school aged adolescents is major public and mental health concern in Ghana and other developing countries, with the rate of usage soaring high among school going adolescents. The objective of this study was to investigate the prevalence of marijuana utilization among selected Junior High School (JHS) students in the Central Region of Ghana and explore the relative impact of psychosocial factors accounting for its usage. Using a descriptive cross-sectional survey design with the Global School Based Survey GSHS questionnaire, a sample of 1400 school going adolescents students were drawn using multistage sampling procedure. Frequencies, percentages and binary logistic regression results indicated marijuana utilization prevalence of 9% (n = 122). Statistically, gender (OR = 0.52, 95% CI= 0.35 - .765, p = 0.001), religious affiliation (OR = 1.76, 95% CI = 1.0 - 2.95, p = 0.034), socioeconomic background (OR = 0.52, 95% CI = 0.33 - 1.23, p = 0.004) and geographical location (OR = 0.53, 95% CI = 0.31 - .886, p = 0.016) significantly predicted marijuana utilization among school aged adolescents. No statistically significant variations were found in the odds of students’ marijuana usage for age (OR = 1.15, 95% CI = 0.69 - 1.88, p = 0.590), parental communication (OR = 0.83, 95% CI = 0.56 - 1.23, p = 0.348) and academic performance (OR = 1.09, 95% CI = 0.66 -1.80, p = 0.744). Findings suggest that school based research should reflect and perhaps replicate existing prevalence, patterns of marijuana and other drug use through multiple school surveys nationwide. This pathway may provide useful information towards the design, evaluation and implementation of drug prevention cognitive-behavioural interventions and the development of stringent drug regulative standards.

Physico-Chemical and Bacteriological Quality of Water from Boreholes in Otuoke Community, Bayelsa State, Nigeria

The concern over exposure to drinking water contaminated and the resultant adverse effect on human health has prompted several studies evaluating the quality of drinking water sources. This study was carried out to determine the bacteriological and Physico-chemical qualities of commercial borehole water within Otuoke community in Bayelsa state Nigeria. Ten (10) water samples were collected from the various locations designated as (A-J). The Physico-chemical parameters were determined by using the photometric technique through the Colour Q photometer and the bacteriological analysis was determined by using aerobic plate method. The Physico-chemical parameters of the water samples analysed were within the acceptable limit of WHO standard of drinking water quality except for Bromine and cyanuric acid which ranged from 1.6±0.1 to 2.7±0.1 and 2±1 to 9±1 respectively. The Result of total bacteria count obtained from the borehole water samples ranged from 1.1 x 105cfu/ml to 6.9 x 105cfu/ml and there was no detected growth for the faecal coliform count of most of the samples except for BHW-I and BHW-J which had 1.3 x 104 to 7.9 x 104 respectively. Bacteria isolated and identified using conventional biochemical test include E. coli (17.9%), Klebsiella spp. (35.7%), Salmonella spp(25.0%), Enterobacter aerogenes (21.4%) The study therefore suggests that all the borehole water tested in Otuoke and environs are considered unsafe for consumption and therefore regular treatment before usage is recommended.

Rare Lipomatous Neoplasm of The Thigh in A 13 Year Old Male with A Discussion of Imaging Features and Differential Diagnosis of A Fatty Extremity Mass

Lipomatous tumors are among the most common primary musculoskeletal neoplasms affecting both pediatric and adult patient populations. Patient age, tumor location, and imaging features all contribute to the differential diagnosis of musculoskeletal tumors. Tumors identified outside of common patient demographics or in unusual locations may lead to preoperative misdiagnosis. We present an uncommon adipocytic tumor occurring at an uncommon age which was proven at surgery to represent a preoperatively unexpected diagnosis. A 13 year old male presented with a fatty anterior proximal thigh mass; age and magnetic resonance findings suggested lipoblastoma. However, following complete surgical resection, histopathology confirmed hibernoma, a benign lipomatous tumor characterized by the presence of white and multivacuolated brown fat cells, the vast majority of which occur in adult patients.

Health Literacy and Older Adults: Fall Prevention and Health Literacy in a Midwestern State

Falls continue to be a health concern for older adults. Gender and age are key predictors of falls, particularly for those with low health literacy. Data were collected by a State Health Department using the state-led supplemental question option for the Centers for Disease Control and Prevention (CDC) Behavior Risk Factor Surveillance System (BRFSS) survey. Specifically, trend data for respondents were examined by gender, geographic location and health literacy rate. Results indicated the highest mean number falls occurred with men for those within the metropolitan statistical area (MSA) city code, lowest in suburban areas. Females reported to have a higher estimated mean number of falls and injuries than males. For males, the number of falls and injuries tended to decrease with higher health literacy. The findings support the importance of addressing fall prevention for older adults. Long term implications for improving fall prevention and health literacy for older adults include impact on individual outcomes.

Evaluation of an External Quality Assessment Program for HIV Testing in Tigray North Ethiopia 2016

Background: Point-of-care diagnostic tests (POCTs) are increasingly used in both developing and developed countries. They allow same day testing and treatment at remote locations where no laboratory support is available. Quality control measures, which are routinely used in laboratories, have not been widely implemented for POCTs. This aimed to assess the integrity of the entire laboratory testing process, and aims to educate and improve performance in quality of HIV rapid testing. Methods: A health facility based cross section study was conducted from April to June 2016.Randomly selected health facilities were participated in the external quality assessment. Onsite evaluation and panel test were used to collect data using structured checklists and formats. Data was entered and analyzed using SPSS version 16. Results: Between April to June 2016, a total of 60 health facilities (145 testing points) from governmental health facilities (hospitals and health centers) were participated in the study. Among the participated testing points 41% have no designated area, 40% have no clean water for hand washing and 51% have no national test algorithm. The average performance of testing points was varies from 89.6% to 99.1% (Laboratory 99.1%, ANC 90.4%, TB clinic 91.4% and VCT 89.6%). In a multivariable logistic regression model, didn’t follow national testing algorithm to report client test results have statistical significance. Conclusions: High quality test results underpin accurate diagnosis and appropriate treatment for patients. But in the study area the score of proficiency testing result and coverage of training is slightly low comparing to other findings. Therefore following national testing algorithm to report client test results, training and monitoring are critical points to improve the proficiency testing score of testing points.

Overcoming Recruitment Challenges for Securing a Survey Sample of Caregivers of Community-Dwelling Older Adults with Multiple Chronic Conditions

This paper discusses the challenges in recruiting a diverse sample of caregivers of older adults with multiple chronic conditions in two Canadian provinces – Ontario and Alberta, as well as the successful strategies to overcome these challenges. Four strategies were employed to overcome the recruitment challenges experienced. These included: (1) using a multi-pronged recruitment strategy with persistence; (2) knowing the location of the sample in the community; (3) weekly reporting by team members primarily responsible for recruitment and data collection, and; (4) regular team meetings to build on successful recruitment strategies while troubleshooting new approaches. These four strategies complemented one another. The most effective strategies in Alberta (recruiting over 50% of participants) were newspaper advertisements and working with the Alberta Caregivers Association and the Alzheimer Society of Alberta/NWT. In Ontario, the most effective strategy was door to door flyers (recruiting 61.5% of participants).

Statistical Analysis of Malignant Brain Neoplasms (ICD-10: C71) in the Lower Silesia Region of Poland in the Years 2006-2012

It is crucial for the society, the government and the medical community to retrieve the full and reliable statistical information on malignant brain neoplasms (C71-C71.9 ICD-10) to adjust the medical financing, staff and equipment properly. In order to retrieve information useful for public health policies, data from the years 2006-2012 concerning relevant cases registered by the Polish public healthcare insurance provider Narodowy Fundusz Zdrowia in Lower Silesia region of Poland (NFZ) and by the Polish national neoplasms registry Krajowy Rejestr Nowotworow (KRN) were analyzed. The number of new malignant brain neoplasms cases registered by the KRN has risen slightly in the years 2006-2012. At the same time the number of cases reported by the NFZ rose dynamically, which means a significant increase in medical care intensity, and thus also workload on the medical facilities and stuff associated with the care for grossly the same amount of brain malignant neoplasms patients and, supposedly, their longer survival times. It indicates that the level of public financing of the malignant neoplasms of brain treatment shall be adjusted adequately. The study revealed growing popularity among reporting Polish physicians of the least specific malignant neoplasms of brain ICD-10 categories, despite the rapid diagnostic techniques development and availability. It is alarming since the medical statistics data quality in the field of malignant brain neoplasms is deteriorating that way and proper evaluation of treatment costs and planning future financial allocations by both the public healthcare insurance provider NFZ and the Polish government becomes difficult.

Chronic Pain One to Five Years after Lung Transplantation

Chronic bodily pain after lung transplantation has received little attention. Therefore, the aim was to provide a multidimensional assessment of self-reported chronic pain 1-5 years after lung transplantation and its relationship with self-reported psychological general well-being (PGWB) and self-efficacy. This multicenter, cross-sectional study is a part of the Swedish national study: Self-management after thoracic transplantation (SMATT). In total, 117 lung transplant recipients, all white, due for their yearly follow-up at one (n=35), two (n=28), three (n=23), four (n=20) or five years (n=11) after transplantation were included. Of these, 113 reported their pain on the Pain-O-Meter (POM), which provides information about pain intensity, quality, location, and duration. In addition, they responded to the PGWB instrument and the Self-Efficacy instrument for managing chronic disease. The prevalence of pain was 51% after 1 year, 68 % after 2 years, 69.5 % after 3 years, 75 % after 4 years and 54.5 % after 5 years. Women experienced higher pain intensity and worse sensory and affective burden than men. Psychological general well-being was the main factor that contributed to the experience of pain. Better perceived psychological well-being lowered the odds for pain, while higher self-efficacy reduced the probability of experiencing pain. Many of the lung recipients lacked pain treatment and were uncertain about the reasons behind their pain. Chronic bodily pain is a common and serious symptom up to five years after lung transplantation. Female lung recipients experience more pain and pain related illness than men.

Where Do You Sit in Class? A Study of Spatial Positioning During Two Courses of Different Duration

The objective of this study was to study the selection of seat location by individuals in a group in a confined environment and to identify the factors leading people to prefer one location to another. We analyzed the seating location of students in a lecture hall over the course of two academic programs of different durations (19 days and 44 days). The goal was to determine the rate at which participants would settle into a specific seat location. Unobtrusive photography was used to collect objective data on an hourly basis. Results showed that in both courses participants began to settle into a specific location from the second day of class. Twenty percent of the participants had settled after 4-7 days or 15.5 hours in class. Settling continued for the duration of the shorter course. However, in the longer course settling stopped after 28.5 days on average. The plateau in the number of settlers depended on the number of days, not on the time actually spent in class. At the end of the longer course 52.5% of the participants had settled, compared to 38.9% in the shorter course. Settling into the same seat location can be interpreted as a strategy to establish a personal territory. These results indicate that about half of a cohort expresses the need for establishing a personal territory when in a confined and crowded environment, and this process takes about one month.

Evaluation of Household Radiation Exposure and Safety after Ambulatory Radioiodine Ablation Therapy

Objective: Radiation exposure to the general public and patients undergoing diagnostic or therapeutic procedures is of great concern, especially to the medical community. Revision of Nuclear Regulatory Commission rules several years ago yield new recommendations for the administration of therapeutic doses of 131-Iodine that included the release criteria. The guidelines for ambulatory treatment included patient education and radiation safety measures to minimize exposure and contamination. Our goal in this study was to evaluate patient compliance with the radiation safety instruction protocols given to them before the therapeutic dose and monitor radiation levels in different house areas at different times after an ablation therapy of 3700MBq or more. Method: Patients with well differentiated thyroid cancer being evaluated for ablation therapy with 131-Iodine were invited to participate. A thorough set of instructions on radiation protection were given verbally and in writing. Patient house was assessed with a Geiger Muller detector at 24 and 72 hours or above to obtain direct radiation levels in several areas. Patient radiation levels were also monitored. Results: A total of 12 patients have been included, 11 females and 1 male, median age was 53 years. Tumor histology was 10 papillary, 2 papillary-follicular variant and 1 follicular carcinoma; 92% of the cases were T1, N0, M0. Home location was urban in 77% and rural in 33% of the patients; 67% of the patients had an educational level between 9-12 grade. Radioiodine doses range from 3441-5994MBq. None of the patients had a relatives or companion in the house. Mean patient exposure 24 hours after the dose at 1 meter was 12mrem/hr, 0.120mSv/h; this represented a retained dose of 2181MBq (59mCi). Only one patient (T1, Nx, M1) had an exposure rate at 1 meter of 100mrem/hr (1mSv/hr) at 24 hours. At 72 hours the exposure changed to 4mrem/hr, 0.040mSv/hr, retained dose of 725.2MBq (19.6mCi). Higher exposure rates in the house were at 24 hours in the bed and pillows (7mrem/hr), kitchen trash (13mrem/hr) and bathroom sink (8mrem/hr). The exposure rates at the toilet and shower were similar (3-4mrem/hr). There was a significant decreased in the exposure rate at 72 hours in all house areas. Patients with the higher exposure rates were those with metastatic disease, and small living facilities. Patients living in rural or urban location had no difference in the exposure rate. The educational levels were not related either to the exposure rate. Conclusion: Patient compliance with radiation protection instructions and Nuclear Regulatory Commission release criteria was good. Radiation exposure levels in the house areas are safe. Special instructions must be design to minimize contamination in the bathroom and kitchen Caution is recommended in the release of patients with extensive metastatic disease and doses of 5920MBq or more.

Cytoplasmic Retention of CDC6 Induces Premature Senescence in Immortalized Cells and Suppresses Tumor Formation in Mice

Senescence is a powerful mechanism that prevents the development of tumors in vivo; however, once tumors are formed, most are refractory to senescence in response to oncogenic stress. Therefore, a novel pathway leading to senescence is required. We herein demonstrated that the cell cycle regulator CDC6 translocated from the nucleus to the cytoplasm during senescence in a leptomycin B-resistant manner. In order to evaluate the translocation of CDC6, we utilized an estrogen receptor (ER) tag to retain CDC6 in the cytoplasm. ER-tagged CDC6 was exclusively cytoplasmic, inhibited cell proliferation, and induced senescence-associated (SA) b-galactosidase activity. Furthermore, ER-CDC6 inhibited the transformation of mouse fibroblasts by the active ras oncogene in vitro, and suppressed tumor formation in NOD-SCID mice. Thus, CDC6 may play a critical role in the regulation of senescence in the cytoplasm in order to counteract tumorigenesis.

Molecular Cytogenetic Investigations in a Novel Chromosomal Abnormality of t(10;15)(q22;q22) in a Pediatric Precursor-B-Acute Lymphoblastic Leukemia Patient

Acute lymphoblastic leukemia (ALL) is a rapid form of leukemia characterized by clonal proliferation and accumulation of immature hematopoietic stem cells of the lymphoid lineage in the bone marrow as well as peripheral blood. Chromosomal aberrations identified in childhood ALL have an important role in disease diagnosis, prognosis and management. We present the results of hematologic, immunophenotypic, cytogenetic, FISH and Multiplex RT-PCR analysis of a 6-year-old boy diagnosed with B-cell precursor Acute Lymphoblastic Leukemia (BCP- ALL). In this study, we identified a novel chromosomal translocation t(10;15)(q22;q22) by cytogenetic and FISH analysis. To the best of our knowledge, this is the first report of this novel chromosomal translocation in this subset of ALL and has not yet been reported elsewhere. This rearrangement may include certain cancer associated tumor suppressor gene(s) or genes involved in apoptosis and transcription regulation, which on loss of normal function may lead to leukaemogenesis.

Demographics, Clinical Profile and Outcome among the HIV Infected Persons Hospitalized in the HAART Era in Barbados.

Background: HAART has resulted in significant decline in morbidity and mortality from HIV. However, it is unclear if the trends have continued in the current HAART era. An understanding of healthcare utilization patterns is important for optimization of care and resource allocation. We examined the diagnoses for hospitalizations of patients with HIV and their clinical and demographic profile years after the introduction of HAART. Methods: A retrospective audit of the HIV admissions from July 2009 through June 2010. The case notes of all the adult admissions where one of the discharge diagnoses was HIV infection was reviewed. Data including the demographics, date of diagnosis, treatment and the follow up details, admission outcome and the final diagnosis were extracted from the case notes. Results: Over the 12 months period there were 154 admissions where one of the discharge diagnosis was HIV infection, and this accounted for 2.9% of all medical admissions in adults. 103(67%) admissions were in persons who were known to be HIV infected prior to the current admission. HIV infection was diagnosed for the first time during the current admission in 51(33%) cases. Nearly two-thirds of those hospitalized, had a CD 4 cell counts of < 200/µL and 63 (66%) had a viral load greater than 50,000 copies/ml. Over all, opportunistic infection was the commonest (47%) discharge diagnosis, followed by serious bacterial infections and HIV nephropathy. The median duration of hospitalization was 6 days (Range 2 to 71 days). There were 49 (32%) deaths. Conclusions: A significant proportion of patients admitted with HIV infection were still diagnosed on admission and were found to be severely immunosuppressed. An opportunistic infection continues to be the commonest discharge diagnosis in HIV infected patients.
